摘要
目的:对比喙锁韧带重建术与锁骨钩钢板治疗NeerⅡb型锁骨远端骨折的疗效。方法:选择2022年2月—2024年2月南京大学医学院附属苏州医院收治的NeerⅡb型锁骨远端骨折患者78例作为观察对象,采用随机数字表法分为两组,各39例。对照组实施锁骨钩钢板治疗,试验组实施喙锁韧带重建术。比较两组手术效果。结果:试验组手术时间、切口长度长于对照组,术中出血量比对照组多,骨折愈合时间早于对照组(P<0.001)。两组住院时间对比,无统计学差异(P>0.05)。术后3个月,两组患侧疼痛评分降低,且试验组低于对照组(P<0.001);两组肩关节前屈、后伸、外展活动度高于术前,且试验组高于对照组(P<0.001)。结论:与锁骨钩钢板相比,喙锁韧带重建术治疗NeerⅡb型锁骨远端骨折的效果更好,可减轻患者患侧疼痛程度,提高肩关节活动度,促进骨折愈合。
Objective:To compare the therapeutic effects of coracoclavicular ligament reconstruction and clavicle hook plate in the treatment of Neer typeⅡb distal clavicle fractures.Methods:A total of 78 patients with Neer typeⅡb distal clavicle fractures admitted to the Affiliated Suzhou Hospital of Nanjing University Medical School from February 2022 to February 2024 were selected as the observation objects.They were divided into two groups according to random number table method,with 39 patients in each group.The control group received treatment with clavicle hook plate,while the experimental group received coracoclavicular ligament reconstruction.The surgical effects were compared between the two groups.Results:The operation time and incision length in the experimental group were longer than those in the control group,the intraoperative blood loss in the experimental group was more than that in the control group,and the fracture healing time in the experimental group was earlier than that in the control group(P<0.001).There was no statistically significant difference in the length of hospital stay between the two groups(P>0.05).At 3 months after surgery,the pain scores at the affected side decreased in the two groups,and the score in the experimental group was lower than that in the control group(P<0.001).The ranges of motion of shoulder joint anterior flexion,posterior extension,and abduction in the two groups were higher than those before surgery,and these parameters in the experimental group were higher than those in the control group(P<0.001).Conclusion:Coracoclavicular ligament reconstruction is more effective than clavicular hook plate in the treatment of Neer typeⅡb distal clavicle fractures,and can relieve the pain at the affected side of patients,improve the range of motion of shoulder joint,and promote fracture healing.
