摘要
目的旨在比较NeerⅡ型锁骨远端骨折分别采用带线锚钉结合锁骨远端锁定钢板和锁骨钩钢板内固定手术的治疗效果。方法本科于2019年1月1日~2022年12月31日对48例NeerⅡ型锁骨骨折患者行内固定手术,A组行锁骨钩钢板固定,共24例;B组行锁骨远端锁定钢板结合带线锚钉固定,共24例。对比两组患者的手术时间、出血量、住院时间等指标,以及术后6个月时患肢的疼痛VAS评分、肩关节功能Constant-Murley评分,并记录术后6个月内并发症发生情况。结果B组患者的手术时间、术中出血量高于A组,差异均有统计学意义(P<0.05),两组住院时间无明显差异(P>0.05);术后6个月,B组VAS评分与Constant-Murley评分改善情况均显著优于A组(P<0.05);术后6个月内,两组患者的并发症总发生率差异无统计学意义(P>0.05)。结论与锁骨钩钢板相比,锁骨远端锁定钢板结合带线锚钉固定方式治疗NeerⅡ型锁骨远端骨折,虽然在手术时间和术中出血量方面不占优势,但可有效降低术后疼痛、恢复肩关节功能,具有更好的临床疗效。
Objective To compare the efficacy of distal clavicular locking plate with suture anchors technique versus clavicular hook plate technique in the treatment of Neer II clavicular fractures.Methods A total of 48 patients with Neer II clavicular fractures between January 2019 to December 2022 underwent internal fixation surgery.The patients were divided into two groups:Group A which underwent fixation with a clavicular hook plate(24 cases),and Group B which underwent fixation with a distal locking plate combined with suture anchors(24 cases).The perioperative parameters such as operation time,blood loss,and hospital stay were compared between the two groups.Visual analog scale(VAS)and Constant-Murley shoulder score at 6 months postoperatively were compared,and complications occurring within 6 months after surgery were recorded.Results The surgical time and intraoperative blood output of patients in Group B were higher than those in Group A,with statistical significance(P<0.05).There was no significant difference in hospital stay between the two groups(P>0.05);six months after surgery,the improvement of VAS score and Constant-Murley score in Group B was significantly better than that in Group A(P<0.05);within 6 months after surgery,there was no statistically significant difference in the total incidence of complications between the two groups of patients(P>0.05).Conclusion In the treatment of Neer II clavicular fractures,the distal clavicular locking plate with suture anchors technique may not have advantages in terms of operation time and intraoperative blood loss.However,it can effectively reduce postoperative pain,improve shoulder joint function,and achieve better clinical outcomes.
