摘要
目的探讨不同浓度高渗葡萄糖增生疗法对肩袖损伤的疗效。方法将60例肩袖损伤患者随机分为A、B、C 3组(每组各20例),分别接受10%、20%、25%葡萄糖溶液,在肌骨超声引导下进行冈上肌腱表面注射,每周1次,连续3次。治疗前后采用尼尔试验(Neer)肩关节功能评分评估疼痛、活动度、力量和日常功能。结果3组患者治疗前Neer评分比较差异无统计学意义(P>0.05)。治疗后1个月、2个月、3个月,3组患者Neer肩关节功能评分均显著提升(P<0.05),其中B组、C组患者改善程度优于A组(P<0.05),B组与C组间比较,差异无统计学意义(P>0.05)。常见不良反应为延迟性疼痛,C组患者不良反应发生率高于其余2组(P<0.05)。结论高渗葡萄糖增生疗法可有效改善肩袖损伤症状,疗效随浓度增加而增强,20%葡萄糖浓度在疗效与安全性间达最佳平衡。
Objective To evaluate the therapeutic effects of different concentrations of hypertonic glucose therapy on rotator cuff injuries and identify the optimal concentration.Methods Sixty patients with rotator cuff injuries were randomly assigned to three groups(Group A,B,and C,n=20).They received 10%,20%,and 25%glucose solutions in group A,group Band group C,respectively,and via supra-supraspinatus tendon surface injections under musculoskeletal ultrasound guidance.Injections were administered weekly for three consecutive sessions.Pain,range of motion,strength,and daily function were assessed pre-and post-treatment using the Neer shoulder function score.ResultsPre-treatment scores among the three patient groups showed no statistically significant differences(P>0.05).At 1,2,and 3 months post-treatment,scores significantly improved in all three groups(P<0.05).Groups Band C demonstrated superior efficacy compared to Group A(P<0.05),while no statistically significant difference was observed between Groups Band C(P>0.05).Delayed pain was the most common adverse reaction.occur ring at a higher rate in 25%of patients in this group compared to the other two groups(P<0.05).ConclusionHyperosmotic glucose therapy can effectively improve the symptoms of rotator cuff injury,and the efficacy increases with increasing concentra tion.A 20%glucose concentration achieves the optimal balance between therapeutic effect and safety.
