摘要
探讨内皮素与高血压胰岛素抵抗的相关性和卡托普利加左旋氨氯地平对高血压患者胰岛素抵抗的改善作用。选择在本院内科就诊的25例原发性高血压伴糖耐量异常患者,使用卡托普利加左旋氨氯地平联合用药治疗,在治疗前与治疗后测量收缩压、舒张压,测定空腹血糖、胰岛素、胰岛素抗体和胰岛素抵抗指数的变化。对照组21例为体检健康人群。高血压组血浆ET-1水平均显著高于对照组(P<0.01),ET-1与FPG、FINS、Anti-INS、HOMA-IR呈正相关。高血压患者治疗后收缩压、舒张压、空腹血糖、胰岛素、胰岛素抗体和胰岛素抵抗指数均显著降低,与治疗前比较均有显著差异(P<0.05或P<0.01)。内皮素与高血压胰岛素抵抗指标有良好的相关性,卡托普利和左旋氨氯地平对高血压患者胰岛素抵抗有明显的改善作用。
To explore the relationship between plasma endothelin and hypertension insulin resistance,and the improvement of insulin resistance in hypertension patients treated with captopril and l-amlodipine,25 patients with primary hypertension and impaired glucose tolerance were selected and treated by captopril and l-amlodipine.Systolic pressure,diastolic pressure, fasting blood glucose,insulin and insulin antibody were measured before and after treatment and compared with healthy controls.The results showed that the plasma ET-1 level in hypertension group was significantly higher than that of healthy controls（P0.01）,and he plasma ET-1 level was positively correlated with FPG,FINS,Anti-INS,HOMA-IR.The systolic pressure,diastolic pressure,fasting blood glucose,insulin,insulin antibody and insulin resistance index in hypertension patients were decreased significantly after treatment（P0.05）.There is a good correlation between endothelin and insulin resistance index in hypertension patients.Captopril and l-amlodipine had obvious improvement effect on insulin resistance in hypertension patients.
