摘要
摘要:目的 探讨三维斑点追踪技术对射血分数正常的重度主动脉瓣狭窄患者左室心肌收缩功能的评价效果。方法 选择2015年7月到2017年5月之间在我院诊断为射血分数正常的重度主动脉瓣狭窄患者80例作为病例组,另外选择同时间段内在我医院进行健康体检的患者80例作为对照,利用三维超声斑点技术对患者的心脏进行扫描,对结果进行评价。结果 病例组和对照组患者的左心室舒张末期内径,左心室收缩末期内径,左心室射血分数数据差异不具有统计学意义(P〉0.05),室间隔厚度,左室后壁舒张末期厚度为病例组高于对照组,病例组患者的左心室整体长轴应变,左心室整体径向应变,三维应变低于对照组,左心室整体环向应变,左心室整体面积应变高于对照组,数据差异具有统计学意义(P〈0.05),利用常规心动图和三维斑点追踪技术数据进行患者的判别分析,分类正确率分别为85.31%,100.00%。结论 三维超声斑点追踪技术已经成为临床辅助检查手段之中一种很有价值的方法,在对于射血分数正常的重度主动脉瓣狭窄患者左室心肌收缩功能的评价上有着独特的优势,可以将该部分患者和正常人进行区分,这对该类别患者的诊治有着重大的意义,值得在临床范围内进行推广。
Objective To evaluate the effect of three-dimensional spot tracking technique on left ventricular systolic function in patients with severe aortic stenosis with normal ejection fraction. Methods A total of 80 patients with severe aortic stenosis diagnosed with normal ejection fraction between our hospital from July 2015 to May 2017 were selected as the case group, also selected in the same period in my hospital for physical examination of 80 patients as a eontrol. The heart of the patient was scanned using a three-dimensional ultrasound spot technique, and the results were evaluated. Results The ease group and the control group of patients with left ven- trieular end diastolic diameter[the case group(47.78±3.19)mm, the control group(47.73±2.59)mm ], left ventricu- lar end systolic diameter[the case group(27.05±2.15)mm, the control group(26.87±2.13)mm], left ventricular ejection fraction [ the case group ( 60.49± 1.96 ) %, the control group ( 60.57±2.02 ) % ] data difference was not statisti-cally significant (P〉0.05); Interventricular septal thickness [the case group (12.70±0.85)mm, the control group (7.83+0.84)mm], left ventricular posterior wall diastolic thickness [the case group (10.56±0.73)mm, the control group (7.17±0.69)mm ] were higher than the control group; left ventricular global systolic strain patients [the case group(15.17±1.21)%, the control group(20.15-±1.17)%], left ventricular the overall radial strain[the ease group (27.49±4.73)%, the control group(38.05±5.40)% ], three-dimensional strain[ the case group(32.55±4.77 )%, the control group (41.97±4.50)% ] were lower than the control group, left ventricular global circumferential strain[the case group (30.24±1.72)%, the control group (24.81±1.66)%] and left ventricular area strain [the case group (45.27±1.91)%, the control group(41.47±1.46)% ] were higher than the control group(P〈0.05 ). The discriminant analysis of patients using conventional echoeardiography and three-dimensional speckle tracking technique was 85.31% and 100%. Conclusion Three-dimensional ultrasound spot tracking technology has a unique advantage in the normal ejection fraction of severe aortic stenosis in patients with left ventncular myocardial systolic function evaluation, you can distinguish between some patients and normal people, and the diagnosis and treatment of patients in this category is of great significance, it is worth to promote the clinical scope.
