超声斑点追踪技术测量正常人二维应变的初步研究 被引量:136

Measurements of two dimensional strain in healthy subjects by speckle tracking echocardiography

摘要 目的 运用斑点追踪技术测量正常人左心室各个节段二维应变。方法 对21例正常人行超声心动图检查记录左室短轴观二尖瓣、乳头肌、心尖部图像和心尖四腔观、二腔观、左室长轴观高帧频图像,应用二维应变分析软件测量各节段的二维应变,同时应用定量组织速度成像技术测量心尖位纵向应变,检验二者对同一部位应变测量的重复性。结果 二维应变所测得心尖位左室基底段、中间段、心尖段的纵向应变依次递增,基底段最小,心尖段最大。二维应变测量重复性高,心尖位变异系数观察者内为1.5%,观察者间为1.8%;短轴变异系数观察者内为6.1%,观察者间为14.2%;定量组织速度成像技术测量纵向应变变异系数观察者内为65.1%,观察者间为78.3%。结论 应用斑点追踪技术测量心肌节段二维应变不依赖室壁运动的方向,具有较高重复性,其在各种心脏疾病中的应用价值有待进一步研究。 Objective To evaluate the two-dimensional strain by speckle tracking echocardiography in healthy subjects. Methods Study population consisted of 21 healthy subjects (male 14,female 7) with mean age of (29.5 - 10.5) years. High frame rate two-dimensional images were recorded from the left ventricular short-axis views at the levels of mitral annulus, papillary muscle and apex, and the apical four-chamber view, two-chamber view and long-axis view of the left ventricle. The longitudinal strain was measured in the apical views. Radial strain, circumferential strain and rotation were measured in the left ventricular short-axis views using two-dimensional strain software. The longitudinal strain in apical views was also obtained by quantitative tissue velocity imaging. The interobserver and intraobserver variabilities were evaluated for the measurement of strain by speckle tracking echocardiography and quantitative tissue velocity imaging. Results Left ventricular two-dimensional longitudinal strain gradually increased from the base to apex. Measurement of two-dimensional longitudinal strain was highly reproducible with 1.5% and 1.8% interobserver and intraobserver variabilities compared with the 65.1% and 78.3% variabilities for longitudinal strain measured by quantitative tissue velocity imaging. The interobserver and intraobserver variabilities were 6.1% and 14.2% for the strain measured by speckle tracking in short-axis views. Conclusions Two-dimensional speckle tracking technique has a good reproducibility and is independent of insonation angle for assessing myocardial strain. Further study is needed to assess its usefulness for the evaluation of heart disease.
