摘要
目的分析宁夏回族自治区宁南地区发病率排名前2位医疗费用的影响因素,比较在不同医疗保险制度下按业务量排名前2位的住院总费用及各结构费用是否存在差异,探讨不同医疗保险的类型作为住院的支付方式对宁南地区居民医疗费用的影响,并探讨不同医保类型对患者能否造成就医经济负担。方法通过获得宁南地区病案数据库得到当地发病率最高的前2种疾病,采用卡方检验和方差分析比较不同类型医保患者的性别、民族、年龄、住院天数的分布情况、住院总费用及各结构费用的构成情况,应用多元线性回归模型分析宁南地区居民医疗费用的影响因素。结果城镇职工医保、城镇居民医保和新型农村合作医疗的次均费用普遍高于自费患者的次均费用,4种不同医疗保险类型患者的性别、民族、年龄和住院天数构成率存在差异;住院总费用和各结构费用两两比较存在差异。建议(1)控制疾病医疗费用增长;(2)改进基本医疗保险方案设计。
OBJECTIVE Analyze incidence of the Ningxia autonomous region building area in the top two influence factors of medical expenses, compare under different medical insurance system according to the volume of the top two hospital total costs and fees if there was a difference of each structure, explore the types of different medical insurance as payment for building area residents in hos- pital medical expenses, and to explore different kinds of health care for the patient can cause medical economic burden. METHODS Through access to medical record database to get the highest incidence of local building area before the two diseases, using chi-square test and variance analysis and comparison of different types of medicare patients gender, nationality, age, and the distribu- tion of hospitalization days and total hospitalization expenses and expenses of each structure composition, using multiple linear regres- sion model to analyze the influence factors of residents＂ medical expenses building area. RESULTS For urban workers, urban resi- dents and all costs of the new type of rural cooperative medical care is generally higher than at the patient＇s second all fees, four types of without medical insurance patients gender, nationality, age and hospitalization days rate differences; Total hospitalization ex- penses and costs of each structure comparing two differences. SUGGESTION （1） Control the growth of medical expenses for illness （2）Improve basic medical insurance scheme design.
