摘要
目的 进一步认识氟西汀与阿米替林治疗抑郁症的疗效和副作用差异。方法 收集 1 7篇主要在国内精神科杂志上公开发表的关于氟西汀与阿米替林对照研究的文章 ,取其研究结果作 Meta分析。结果 氟西汀治疗前后自身对照比较 :综合检验 Z1 =50 .97,P<0 .0 1 ,提示氟西汀治疗抑郁症前后症状学变化有非常显著性差异 ;d1 =4 .4 2 ,表明这种治疗效应极大。与阿米替林对照组的组间比较 :Z2 =-1 .2 0 ,P>0 .0 5,提示两种药物的疗效没有显著性差异 ;d2 =0 .1 0 ,表明这种差异效应极小。治疗后副作用综合检验 Z3=1 2 .96,P<0 .0 1 ,提示氟西汀的副作用显著少于阿米替林 ;d3=1 .90 ,表明两者差异效应极大。氟西汀组恶心呕吐多于阿米替林组 ,差异有显著性 ( P<0 .0 1 ) ,强度介于轻与中度之间 ( d=0 .33)。其余 8种副作用氟西汀组显著低于阿米替林组 ( P<0 .0 1 ) ,强度较为明显 ( d=0 .34～ 1 .0 7) ,其发生的比值比在 3.5～ 7.2之间。结论 氟西汀与阿米替林在 6周左右的疗效无差异 。
Objective To study the difference in therapeutic effects and side effects between fluoxetine(Flu)and amitriptyline(Ami).Method Meta analysis was made for 17 papers published about comparative study of Flu and Ami.Results The significant difference in pre-and post-treatment of Flu group was found(Z 1=50.97,P<0.01),and its threrapeutic effect size was 4.42( 1=4.42).The incidence of delygmice and vomit in group Flu was significantly different from group Ami(χ 2= 11.848 ,P<0.01),and its effect size was between middle-great.The other side effects were lower in group Flu than that in group Ami(χ 2=16.722～135.765,P<0.01).The effect size of other side effect in group Ami was 0.34～1.07 with high level.Conclusion Fluxetine difference from amitriptyline is in side effects,but not in therapeutic effects.
