子宫内膜轻创术对IVF-ET着床失败再次治疗结局的影响 被引量:2

Effect of endometrial debridement on clinical outcome in the subsequent cycle in patients with implantation failure in previous IVFET

摘要 目的探讨子宫内膜的轻创术对体外受精-胚胎移植(IVF-ET)着床失败患者再次治疗临床结局的影响。方法选取84例有着床失败史的IVF-ET患者,随机分为三组,A组27例移植周期月经第1~3天间行子宫内膜轻创术,B组29例在移植周期前一周期的黄体期行子宫内膜轻创术,C组28例为对照组未行轻创术。统计三组患者的胚胎着床率、妊娠率、流产和宫外孕等方面差异。结果轻创术后A、B组的胚胎着床率分别为39.2%、38.3%,妊娠率分别为63.0%、55.2%,均高于未轻创C组的29.6%、46.4%,但无统计学意义(P均>0.05),出现的2例宫外孕均在A组。结论子宫内膜轻创术选择在上一周期的黄体期施行比较适合,对胚胎着床可能有一定的帮助,但还需进一步研究。 Objective To study the influence of endometrial debridement on clinical outcome in the subsequent cycle in patients with implantation failure in previous IVFET(in-vitro fertilization and embryo transfer). Methods 84 patients who had implantation failure in the previous IVFET were divided into three groups randomly. Group A (27 cases) underwent endometrial debridement from dayl to day3 of menstruation in the transplantation cycle; group B (29eases) underwent endometrial debridement in the luteal phase of the circle previous to the transplantation cycle;28 cases in group C(the control group) were not treated by endometrial debridement. Then,differences in the embryo im- plantation rate, the pregnancy rate and miscarriage and ectopic pregnancy of the three groups were analyzed and com- pared. Results After the endometrial debridement, the embryo implantation rates of group A and group B were 39.2 % and 38.3 %, respectively, and their pregnancy rates were 63.0 % and 55.2 %, respectively, which were all high- er than those of group C (29.6 % and 46.4%, respectively). But there were no significant differences for these com- parisons( P 〉0.05). In addition,there are 2 cases of ectopic pregnancy in group A. Conclusion Endometrial debride- ment it's relatively safe to be performed in the inuteal phase of the previous circle,which may benefit embryo implan- tation, but further research is needed.
