机械刺激对种植窗期C型子宫内膜形态学的影响 被引量:3

摘要 目的探讨改善子宫内膜容受性的方法,以提高体外受精—胚胎移植(IVF-ET)成功率。方法选月经自然周期尿黄体生成素(LH)峰日C型子宫内膜不孕妇女20例为研究对象,分为观察组和对照组各10例,观察组在尿LH峰日刮取子宫内膜1次,其后两组均在排卵后第7天局部刮取子宫内膜1次,应用组织形态学方法和图像分析及扫描电镜技术观察、分析子宫内膜组织形态变化及胞饮突特征。结果①光学显微镜下对照组子宫内膜腺腔轻度扩大、见核下空泡,观察组子宫内膜腺腔增大、顶浆分泌旺盛;两组子宫内膜组织结构定量分析指标比较P<0.05。②扫描电镜下观察,对照组子宫内膜的非完全发育胞饮突/完全发育胞饮突为10/0、观察组为4/6,P<0.05。结论卵泡晚期C型子宫内膜在种植窗期存在发育障碍,呈黄体早期表现;尿LH峰日机械刺激C型子宫内膜可促进种植窗期腺体和胞饮突发育。 Objective To explore the. effect of machinery stimulation on endometrium C morphology. Methods Choose 20 infertile women who are in natural cycle urine LH Days as researched objects and divide them into contrasted group and stimulated group. During this, stimulated group scrape endometrium C once in LH Days and stimulated endometrium C. Scrape endometrium C in the 7th day when two groups of endometrium C ovulate, apply organized morphology, picture and Scanning electron microscope(SEM) technology to observe and analyze the changes of organized form and pinopodes character. Results Under the light microscope, contrasted groups see endometrium slightly enlarged, find the empty bubble under the nuclear and take on early secrective indication; the stimulated groups finds endometrium gland cave bigger and secreting thrively. The picture analytic index shows gland area ,gland perimeter, galandular maxium daimeter, area of gland cavity, circumference of gland cavity, the biggest diameter of gland cavity, total area of glandular epithelial cell, average height of glandular epithelial cell, and the constituent rate index of endometrium organizing structure, gland area, area of gland cavity, total area of glandular epithelial cell, stromal area and glandular area/stromal areas, two groups to compare with each other ,they will be prominent in their compared difference. With the SEM of 120 times observing, surface of endometrium C of contrasted groups gets frown, uneven, the gland seldom bursts, the cutting of the gland is tiny and conceals the frownly concave place of the endometrium. The undeveloped pinopodes and developed pinopodes of the stimulated endometriua's surfaces are flatter, large amount of gland bursts, big and round, and stans out from the surface of the endometrium. The undeveloped pinopedes and the developed pinopodes are 10/0, but the undeveloped pinopodes and the developed pinopodes of the stimulated group endometria are 4/6, the compared difference of two groups is prominent, without any degenerated pinopodes. Conclusions Endometrium C of late follicular phase exists developed barrier during the window of planting and appears the early period of corpus luteum. In urine LH Days ,machinery stimulated endometrium C can promote all kinds of glands during the window of planting and pinopodes to develop.
