摘要 目的:动态观察脑出血(CH)及蛛网膜下腔出血(SAH)患者血浆神经肽Y(NPY)的水平变化,探讨NPY与出血性脑血管病发生、发展的关系。方法:采用放射免疫法测定了46例脑出血,30例SAH患者及28例健康人血浆NPY的含量。结果:CH和SAH患者血浆NPY水平均明显高于对照组(P<0.001)。发病24小时内即显著升高,4~7天达高峰,8~15天开始下降,15天后CH组仍维持较高水平。重型CH患者NPY水平显著高于轻型和中型。大面积出血组高于小面积出血组。SAH患者伴迟发性缺血性障碍(DID)组明显高于不伴DID组。结论:NPY可能在CH及SAH的发生及病理生理过程中起重要作用。 Objective: To observe dynamically the plasma Neuropeptide Y(NPY) concentration in patients with cerebral hemorrhage (CH) and subarachnoid hemorrhage (SAH) and its clinical significance. Methods: The levels of NPY in plasma were measured by radio immunoassay in 46 cases of CH patients. 30cases of SAH patients and 28 helthys as the normal control. Results: Compared to the control group, the levels of NPY in patients with CH and SAH were increased significantly ( P <0. 001). The levels of NPY began to increase in 24 hours, to reach the peak on days 4-7. After the days 8-15, it began to decrease, but the levels of NPY in patients with CH were still higher than those in control group after 15 day . The changes of NPY were related with the diameter of hemorrhage site and neurological functional scales. There was a marked elevation in NPY levels in patients with SAH complicated by delayed ischemic deficits (DID) compared with whose in patients without (DID) ( P <0. 05). Conclusion: NPY play a important role in the development of hemorrhagic stroke.
