摘要
目的 :探讨多间隙腰椎间盘突出的MRI诊断及手术治疗方法的选择。方法 :98例多间隙腰椎间盘突出症患者均行中立位及屈曲位MRI检查 ,根据不同节段及不同退变形式 ,结合临床及影像学表现 ,采用不同方法手术治疗 83例。结果 :多间隙腰椎间盘突出以L4/ 5、L5 /S1,L3/ 4、L4/ 5双间隙多见 ,分别为 43例 (43 87% )和 2 1例(2 1 42 % ) ,有 6例为L3/ 4、L5 /S1突出 (6 1% ) ,3间隙病变者 19例 (19 38% ) ,4个间隙或 5个间隙者 9例(9 18% )。术后随访 6～ 36个月 ,平均 1 5年 ,优良率 93 9%。结论 :MRI对多间隙腰椎间盘突出的诊断敏感性高 ,动态MRI能对多间隙腰椎间盘突出及其主要致病间隙进行准确分型、定位 。
Objective:To evaluate the efficacy of the MRI in the diagnosis and treatment of the patient with multiple level lumbar disci herniation.Method:98 cases with multiple level lumbar disci herniation were performed dynamic MRI(neutral or flexsion ).Based on level,degeneration or clinical manifestation and MRI,83 cases underwent surgical operation。Result:In these cases double level group at L4/5 associated with L5/S1,and L3/4 with L4/5 was most commanly seen.There were 43 cases(43%) and 21 cases(21%) respectively,L3/4 with L5/S1 were 6 cases(6 1%),3 multiple level herniation was 19 cases(20%),and other 9 cases(9%)had 4 or 5 level disc herniation.83 cases who accepted surgical operation were followed up 6～36 months with average 1.5 years.The excellent and good effective rate was 93 9%.Conclusion:The sensitivity of MRI for diagnosing disc herniation is higher than other methods.It is possible to classify and localize multiple level lumbar disci herniation which of the main lession leads to symptoms and signs by means of dynamic MRI.Thus,it will provide fundation for treatment.
