摘要
目的探讨三间隙腰椎间盘突出症的CT与临床表现。方法回顾16例CT检查证实的三间隙腰椎间盘突出症的CT与临床表现。结果16例三间隙腰椎间盘突出症均位于L3、4、L4、5及L5S1,其椎间盘突出的CT表现与单间隙椎间盘突出相同,特点是由上而下突出程度加重,下位间盘疝的钙化较为常见,而前突型仅见于L3、4椎间盘。结论三间隙腰椎间盘突出症的CT表现无特异性,但其临床表现多样。对年龄较大、临床表现复杂者应进行多间隙CT扫描,以避免漏诊。
Objective To investigate the CT and clinical manifestation of three- level lumbar disc herniation （TLLDH）. Methods CT and clinical manifestations of 16 patients with TLLDH were reviewed. Results TLLDH was found in all 16 patients in L3,4, L4,5 and L5S1. The CT imagine of TLLDH was same as that of one- level lumbar disc herniation. The herniation and calcification became more serious at the lower lumbar disc. The anterior herniation was only found in L3,4. Conclusion There is no specificity for CT manifestation Of TLLDH, but various clinical manifestations can be found. Multi - level lumbar disc scan should be taken for old patients with various clinical manifestations of herniated intervertebral disc in order to avoid missed diagnosis of TLLDH.
