摘要
为探讨腰椎间盘实出患者之髓核分子量级超微结构的形态,作者经对手术证实3例腰椎间盘突出症患者的髓核和1例鲜尸体的髓核应用原子力显微镜,进行了超微结构研究。结果表明两者髓核中的胶原束,胶原纤维和原胶原纤维呈不同排列,蛋白多糖的颗粒分子形态与正常亦不一,胶原纤维形成原胶原纤维网孔附着蛋白分子及蛋白多糖,DNA吸附在纤维细胞核RNA单链上,呈双链、多链三维螺旋结构形态。原胶原纤维呈带状结构(BandedStructure)64~78nm,退变髓核胶原纤维化合并钙化,其含量明显高于正常(P<0.05)。提示腰椎间盘受到长时间的伤害,使胶原纤维损害断裂导致出现扭结现象,失去正常抗阻力。
Inordertoinvestigatethemorphologyofultramicrostructureinmolecularlevelofnucle-usherniations,theauthorsstudiedtheultramicrostructureofthreecasesofnucleusherniationsoflum-barintervertebraldiscwhichwereprovedbyoperationsandonecaseofnormalnucleuspulposusoffreshcorpsebyusingofatomicforcemicroscope.Theresultsshouedthatthearrangementofcolagenbundle,colagenfiberandprocolagenfiberwerediferentbetwenthetwoconditions,andthemorphol-ogyofmolecularparticlesofproteoglycaninnucleusherniationswasdiferentfromthatinthenormalcondition.Intheconditionofnucleusherniations,thecolagenfiberformedprocolagenfiberreticularporousadheringtothemoleculesofproteinandproteoglycan,andtheDNAadheringtothemonochainofRNAinfiberalcelnuclei,intheformofbichainandmultichainthreedimensionshelixstructures.Thebandedstructure(64~78nm)oftheprocolagenfiber,andthecolagenationandcalcificationofthedegeneratednuclenspulposuses,thequantityofwhichwassignificantgreaterthannomal(P<0.05)in-dicatedthatthechronicinjuryoflumbarintervertebralnucleuspulposusesgaverisetotheinjuryandbreakofthecolagenfiberwhichresultedintwist(tangle)phenomenonsothatnucleuspulposuseslosttheirnormalresistancestrength.
