宫颈人乳头瘤病毒第二代杂交捕获法和实时荧光聚合酶链反应检测法的临床应用比较 被引量:5

Comparison of clinical value between hybrid capture Ⅱ and multi-fluorescent polymerase chain reaction in the detection of the high-risk human papillomavirus types

摘要 目的:比较第二代杂交捕获法(HCⅡ)与实时荧光聚合酶链反应技术(real-time PCR)在检测女性生殖道人乳头瘤病毒(HPV)高危亚型中的临床价值。方法:随机选取2011年1~6月我院妇产科行液基细胞薄层涂片技术进行低度鳞状上皮内病变(LSIL)检查,检查结果为未明确诊断意义的不典型鳞状上皮细胞(ASCUS)患者100例,分别采用real-time PCR与HCⅡ检测HPV高危亚型,比较两者的检测结果。对检测HPV阳性的样本或HPV阴性但液基细胞薄层涂片技术(LCT)≥LSIL的妇女采用阴道镜下活组织病理学检查,以病理结果作为验证两种HPV检测的参考标准。结果:共取病理68例。病理结果证实该人群中子宫颈上皮内瘤变(CIN)Ⅲ级5例,CINⅡ级11例,CINⅠ级19例,慢性宫颈炎和鳞状上皮化生33例。该100例患者HCⅡ和real-time PCR检测高危型HPV的阳性率分别为63.0%和71.0%,二者总符合率为82.3%;HCⅡ检测手段对高危宫颈病变患者进行高危HPV筛查的灵敏性、特异性、准确性、阳性预测值、阴性预测值、阳性似然比和阴性似然比分别为79.3%、88.2%、91.2%、10.1%、99.7%、8.9和0.327;real-time PCR以上各指标分别为100.0%、92.5%、86.7%、8.6%、100.0%、8.7和0。结论:Real-time PCR与HCⅡ检测宫颈上皮内瘤变患者高危型HPV有较好的相关性;但real-time PCR检测高危型HPV具有更好的敏感性和特异性。 Objective:To compare real-time polymerase chain reaction(real-time PCR) with hybrid capture Ⅱ(HC Ⅱ) in the detection of high-risk genotypes of human papillomavirus(HPV) in patients with cervical lesion.Methods:100 patients with cervical atypical squamous cells of undetetemined(TCT≥low grade squamous intraepithelial lesion,LSIL) who received treatment in our hospital department of obstetrics and gynecology were randomly selected from January 2011 to June 2011,cervical samples were detected by real-time PCR and HC Ⅱ to detect high-risk HPV subtypes,and the test results were compared.Results:68 cases of pathology were selected who were confirmed with cervical intraepithelial neoplasia(CIN),and there were 5 cases were CIN Ⅲ,11 cease were CIN Ⅱ,19 cases were CIN I,33 cases were chronic cervicitis and squamous metaplasia.The positive rate of high-risk HPV types were tested by HC Ⅱ and multi-fluorescent real-time PCR were 63.0% and 71.0% respectively,the total coincidence rate of them was 82.3%.The sensitivity,specificity,accuracy,positive prevalue,negative prevalue,positive likelihood ratio and negative likelihood ratio of patients with cervical lesions detected by HC Ⅱ were 79.3%,88.2%,91.2%,10.1%,99.7%,8.9 and 0.327 respectively;Those of real-time PCR were 100.0%,92.5%,86.7%,8.6%,100.0%,8.7 and 0 respectively.Conclusion:There are good correlations between HC Ⅱ methods and multi-fluorescent real-time PCR methods in the detection of high-risk HPV type in patients with cervical intraepithelial neoplasia,however,real-time PCR detection of high-risk HPV has better sensitivity and specificity.
