摘要
目的监测卫生部全国细菌耐药监测网成员单位2010年度临床分离非发酵革兰阴性杆菌对常用抗菌药物敏感性。方法各成员单位按照监测方案要求进行目标细菌的收集、鉴定和药物敏感性测定,并上报数据到负责单位;依据2010年CLSI标准,采用WHONET软件(5.6版本)对收集到的监测资料进行复核、分析、总结。结果全国129所医院参加了该次监测工作,共收集到77 984株临床分离非发酵革兰阴性菌的相关数据,居首位的为铜绿假单胞菌,占非发酵革兰阴性菌的41.0%,其次为鲍氏不动杆菌,占35.5%;药物敏感性测定结果显示,对铜绿假单胞菌敏感率>70.0%的抗菌药物有多黏菌素、阿米卡星、哌拉西林/他唑巴坦、美罗培南、妥布霉素、亚胺培南、头孢他啶和哌拉西林;除多黏菌素(敏感率97.2%)和米诺环素(敏感率62.7%)外,鲍氏不动杆菌对其他被测抗菌药物敏感率均<50.0%,对碳青霉烯类敏感率约为45.0%。结论非发酵革兰阴性杆菌多药耐药已成为世界问题,急需加强细菌耐药监测,控制耐药菌的发展与传播,为临床合理用药提供数据支持。
OBJECTIVE To investigate the antimicrobial resistance of nonfermenting gram-negative bacteria from member units of Mohnarin in 2010. METHODS The bacterial collection, identitication and the determination of drug suscebility were performed according to surveillance program, and the data were reported to the units in charge; referring to CLSI guideline 2010, WHONET 5.6 software was adopted to review ,analyze and summarize the collected surveillance data. RESULTS Totally 129 hospitals nationwide participate in this study, the related data of 77 984 nonfermenting gram-negative bacteria were obtained. Among all the isolates, Pseudornonas aerugi- nosa ranked the first place, accounting for 41. 0%, followed by Acinetobacter baurnannii (35. 5%); the drug susceptibility test results showed that the drug susceptibility rates of P. aeruginosa to colistin, amikacin, piperacillin/tazobactam, meropenem, tobramycin, imipenem, ceftazidime and piperacillin were more than 70.0%, no antibiotics except colistin (97.2%) and minocycline (62.7%) were more than 50.0% for susceptibility rates against of Acinetobacter baumannii, and the susceptivility rates of earbarpenems against A. baumannii, were about 45.0%. CONCLUSION Multidrug resistance in nonfermenting gram-negative bacteria has been a worldwide challenge for healthcare management, it is of urgent need to strengthen the surveillance for bacterial resistance and hlock the further development and spread of resistant bacteria, in order to promote judicious antimicrobial use.
