摘要
目的监测卫生部全国细菌耐药监测网成员单位2010年度脑脊液标本分离病原菌的分布与耐药性。方法各成员单位按照监测方案要求进行目标细菌的收集、鉴定和药物敏感性测定,并上报数据到负责单位;依据2010年CLSI标准,采用WHONET软件(5.6版本)对收集到的监测资料进行复核、分析、总结。结果全国129所医院参加了该次监测工作,共收集到1873株脑脊液分离菌株的相关数据,其中革兰阳性菌1132株,占60.4%;革兰阴性菌741株,占39.6%;前3位分离菌种依次为:表皮葡萄球菌、鲍氏不动杆菌、金黄色葡萄球菌,分别占16.8%、10.5%、6.4%;药物敏感性测定结果显示,MRSA、MRSE检出率为66.7%和73.2%,未发现利奈唑胺和糖肽类不敏感金黄色葡萄球菌,未发现利奈唑胺不敏感肠球菌属,耐糖肽类抗菌药物肠球菌检出率<3.0%;肠杆菌科主要细菌对碳青霉烯类耐药率基本<10.0%,但是铜绿假单胞菌和鲍氏不动杆菌对碳青霉烯类耐药率约为30.0%、60.0%。结论全国脑脊液标本中革兰阳性菌约占60.0%,最常见细菌为表皮葡萄球菌;革兰阳性菌耐药状况与同期总体水平相似,但革兰阴性菌耐药率高于同期总体水平。
OBJECTIVE To investigate the distribution and antimicrobial resistance of pathogens isolated from cerebrospinal fluid from member units of Mohnarin in 2010. METHODS The bacterial collection, identification and the determination of drug susceptibility of the target bacteria were performed according to the surveillance programs, then the data were reported to the units in responsibility; according to CLSI guideline 2010, WHONET 5.6 software was adopted to review, analyze and summarize the surveillance data. RESULTS A total of 129 hospital enrolled in this study, the related data of 1873 strains of bacteria isolated from cerebrospinal fluid were collected, including 1132 (60.4~//00) gram-positive isolates and 741 (39.6%) gram-negative isolates. Staphylococ- cus epidermidis ( 16.8~./oo), Acinetobacter baumannii (10. 5%) and S. aurese (6.4%) were the top 3 isolated bacteria. The susceptibility test results showed that the detection rates of MRSA and MRSE were 66. 7% and 73.2%, respectively. No S. aurese isolates resistance to glycopeptide antibiotics and linezolid were found, and no Enterococcus isolates resistance to linezolid were found, the detection rates of Enterococcus isolates resistance to glycopeptide antibiotics were less than 3.0 % ; the resistance rates to carbapenems of a vast majority of Enterobac- teriaceae were generally less than 10.0%, but the resistance rates of Pseudomonas aeruginosa and Acinetobacter baurnannii to carbapenems were about 30.0~ and 60.0~, respectively. CONCLUSION About 60.0~ of the strains isolated from cerebrospinal fluid are gram-positive bacteria, among which S. epidermidis is most eommon~ the resistance levels of gram-positive bacteria are similar to the overall level at the same period, but the resistance rates of gram-negative bacteria are higher than that at the same period.
