摘要
目的研究类风湿关节炎(RA)患者心功能变化及与其他指标的相关性。方法采用超声心动图(UCG)检测68例RA患者心功能变化,并与20例健康对照组(NC)进行比较;观察RA患者临床症状、体征及实验室指标与心功能参数相关情况。结果 (1)68例RA患者与20例健康对照组比较可知,RA组异常率最高的为左室舒张功能下降并主闭(20.59%),其次为单纯左室舒张功能下降(17.65%)。(2)与NC组相比,RA患者舒张早期血流峰值流速(cm/s)/E峰、短轴缩短率(FS,%)、E/A比值均显著降低(P<0.01),舒张晚期血流峰值流速/A峰显著升高(P<0.01)。(3)Spearman相关分析结果显示,RA患者心功能参数E峰与DAS-28、a-AGP、hs-CRP,E/A比值与收缩压,射血分数(EF,%)与RF,每搏输出量(SV)与DAS-28均呈显著负相关(P<0.05)。E峰与IgG、CD4+CD25+Treg,A峰与15 m步行时间、中医症候总积分、舒张压,E/A比值与CD4+CD25+Treg均呈显著正相关(P<0.05)。结论 RA患者存在心功能下降,表现为E峰、短轴缩短率(FS,%)、E/A比值降低,A峰升高;RA患者心功能与DAS-28、症状体征积分、实验室指标相关。
Objective To study the heart function changes of patients with rheumatoid arthritis and its relationship with other indicators.Methods We used echocardiography（UCG）to observe the changes of heart function in patients with RA and compare with normal control group;observe the correlation of laboratory indicators.Results 1.Compared with the normal control group,UCG abnormal heart function test results was 77.94% in RA group and 40.0% in NC group,P0.01.2.Compared with NC group,early diastolic peak flow velocity（cm / s）/ E peak,fractional shortening（FS,%）,E / A ratio in RA group were significantly decreased（P 0.01）,while late diastolic peak flow velocity / A peak was significantly higher（P0.01）3.Spearman correlation analysis showed peak E and a-AGP,hs-CRP,E / A ratio had negative relationship systolic blood pressure and ejection fraction（EF,%）and RF（P0.05）,so was E peak and the IgG,CD4 ＋ CD25 ＋ Treg,A peak and diastolic blood pressure,E / A ratio of CD4 ＋ CD25 ＋ Treg were significantly positively correlated（P0.05）.Conclusion RA patients had decreased cardiac function in E peak,fractional shortening（FS,%）,decreased E / A ratio,and increased A peak increase,which is related to DAS-28,synptems and signs scores and some laboratory indexes.
