摘要
目的探讨骨科手术切口感染的临床特征。方法回顾性分析2200例骨科手术患者,分析手术切口感染率、病原菌特点及相关因素。结果共58例发生手术切口感染,手术切口感染率为2.6%;革兰阴性菌51株占87.9%,革兰阳性菌7株占12.1%;分布最多的前3种细菌为铜绿假单胞菌占32.7%、鲍氏不动杆菌占22.4%和大肠埃希菌占15.5%;手术切口Ⅰ类感染12例占1.0%,Ⅱ类21例占2.9%,Ⅲ类25例占10.1%,差异有统计学意义(2χ=6.55,P<0.05),手术时间<2 h感染5例占0.8%,2～4 h感染27例占1.8%,>4 h感染26例占16.9%,差异有统计学意义(χ2=10.36,P<0.05)。结论骨科手术切口感染以革兰阴性菌为主,并且与切口类型和手术时间有关,应加强手术切口感染监测。
OBJECTIVE To explore the clinical features of ＇surgical incision infection in orthopedics department. METHODS The clinical data of 2200 eases from orthopedics department were reviewed. The infectious incidence of surgical incision, pathogen features, and its related factors were investigated. RESULTS The incision ira 58 cases were infected （2.6%）. A total of 57 strains of pathogens Were isolated, 51 strains （87.9%） of G- bacteria and 7 strains （12.1%） of G＋ bacteria. The three common bacteria were Pseudomonas aeruginosa （31.6 %）, Acineto- bacterbaumannii （22.4M）, and Escherichia coli （15.5%）. 12 cases （1.0%） were infected in I type incision, 21 cases （2.9%） in Ⅱ type, and 25 cases （10.1%） in Ⅲ type, and they contributed significant difference （x2 =6.55, P〈0.05）. 5 cases （0.8%） were infected in the group that operation time was shorter than 2 hours, 27 cases （1.8%） in the group between 2 to 4 hours, and 26 cases （16. 9%） in the group longer than 4 hours, the differences among three groups were statistically significance （x2= 10. 36, P〈 0. 05）. CONCLUSION Gramnegative Bacteria are the common pathogens in surgical incision infection in orthopedics department, and the infectious rate is related to the incision type and operation time. It Should strengthen the monitoring work for infection of surgical incision in orthopedics department.
