The Relationship Between ^(99)Tc^m-MIBI Imaging of Breast Cancer and the Expression of P-gp,GST-π and Bcl-2

摘要 目的探讨乳腺癌99Tcm-甲氧基异丁基异腈(MIBI)显像与乳腺癌组织中P-gp、GST-π、及Bcl-2等蛋白表达的关系。方法对105例未经治疗的乳腺肿瘤患者术前一周内进行99Tcm-MIBI早期相(30 min)与晚期相(180 min)平面显像,对其中59例乳腺癌患者用免疫组织化学的方法检测手术后标本中P-gp、GST-π及Bcl-2的表达水平。结果P-gp阴性表达患者的晚期T/N明显高于P-gp阳性表达患者(t=-2.49,P=0.002,双侧),两者间RI差异也有统计学意义(t=-2.90,P=0.01),而GST-π、Bcl-2阴性与阳性表达者间早晚期T/N比值及RI的差异均无统计学意义。结论(1)99Tcm-MIBI转运分析可以作为体外检测P-gp表达的敏感指标,P-gp高水平表达的乳腺肿瘤组织其晚期相摄取比值及滞留指数明显低于无P-gp表达的肿瘤。(2)乳腺99Tcm-MIBI显像可以在活体上动态观察多药耐药蛋白P-gp的表达水平,用于多药耐药的预测。 Objective To investigate the relationship between ^99Tc^m-MIBI imaging of breast cancer and the expression of P-gp, GST-π, and Bcl-2 in breast cancer tissues. Methods Scanning of ^99Tc^m-MIBI scintiphotography was performed in 105 untreated-patients in early (30min) and later period(180min) respectively. Results The T/N ra- tios in later period with negative expression of P-gp was higher than that with positive expression ( t = 2.49, P 〈 0.01) ,and so in RI( t = 2.90, P 〈 0.01 ), while no significant difference was found in the T/N ratios and RI with the expression of GST-π and Bcl-2 or not. Conclusion 1. The analysis of 99 Tcm-MIBI scinti-photo graphy may be a sensitive index in detecting P-gp expression in vitro. 2. ^99Tc^m-MIBI scintiphotography in breast tumor can observe the level of MDR gene expression vividly in the living and can be used for predicting MDR.
