摘要
目的通过调查临床分离的葡萄球菌对克林霉素的耐药性,指导临床合理使用克林霉素。方法对医院2007年1月至2008年7月分离的葡萄球菌进行克林霉素、红霉素药敏试验和克林霉素敏感性试验(D-试验)的数据进行回顾性分析。结果分离到229株金黄色葡萄球菌、136株表皮葡萄球菌、171株溶血葡萄球菌、25株其他葡萄球菌,其对克林霉素的耐药情况分别是:金黄色葡萄球菌71.6%(164/229)、表皮葡萄球菌66.9%(91/229)、溶血葡萄球菌66.1%(113/171)、其他葡萄球菌60.0%(15/25)。其中D-试验阳性的金黄色葡萄球菌、表皮葡萄球菌、溶血葡萄球菌、其他葡萄球菌分别有48株、29株、17株、2株,阳性率分别是67.6%、54.7%、41.5%、40.0%;D-试验阳性甲氧西林耐药金黄色葡萄球菌和甲氧西林敏感金黄色葡萄球菌分别有29株、19株,阳性率分别是76.3%、59.6%;甲氧西林耐药凝固酶阴性葡萄球菌和甲氧西林敏感凝固酶阴性葡萄球菌分别有37株、11株,阳性率分别是54.4%、35.5%。结论葡萄球菌对克林霉素耐药率高,D-试验阳性率高,检测葡萄球菌时要常规做克林霉素的敏感性试验和D-试验。
Objective To guide clinic appropriate use of the medicine by detection of resistance of clinical dissociative staphylococcus to clindamycin. Methods To review and analyze the data of Staphylococcus separated from Jan. 2007 to Jul. 2008 in our hospital through Clindamycin, Erythromycin drug sensitive testing and clindamycin-in- duced resistance testing(D-test). Results The resistance of 229 strains of Staphylococcus aureus, 136 strains of Staphylococcus epidermidin, 171 strains of Staphylocoecu haemolytic,25 strains of other Staphylococcus to Clindamycin are 164 strains, 91 strains, 113 strains, and 15 strains respectively, with the resistance rates of 71. 6%, 66.9 ~%0, 66.1%, 60.0 % respectively. Among them D-test positive Staphylococcus aureus,Staphylococcus epidermi din,Staphylococcus haemolytic, and other Staphylococcus are 48, 29,17, and 2 strains respectively, which occupy 67.6%, 54.7%, 41.5%, 40.0% of their Erythromycin resistance, Clindamycin sensitive bacterium respectively, while D-test positive Methicillin resistant Staphylococcus aureus (MRSA) and Methicillin sensitive Staphylococcus aureus (MSSA) are 29 and 19 strains, which occupy 76. 3% and 59. 6%, Methicillin resistant Coagulase-negative staphylococcus (MRCNS) and Methieillin sensitive Coagulase-negative staphylococcus (MSCNS) are 37 and 11 stains,which aoccupy 54.40/00 and 35.5% respectively. Conclusion Resistance of Staphylococcus to Clindamyein is high with the D-test positive rate. Thus sensitive test and D-test of Clindamycin should be routinely performed for detection of Staphylococcus.
