摘要
目的探讨胰岛素样生长因子Ⅰ(IGF-Ⅰ)及其受体(IGF-ⅠR)在胃癌的表达情况及临床意义。方法应用免疫组化方法检测40例原发性胃癌组织及其远端正常胃黏膜中IGF-Ⅰ和IGF-ⅠR的表达,并分析其表达与胃癌病理分级、临床分期、淋巴结转移、肿瘤侵袭深度的关系。结果胃癌组织IGF-Ⅰ和IGF-ⅠR表达的阳性率分别为80%、77.5%,均显著高于远端正常胃黏膜的阳性率27.5%、25%(P<0.005)。IGF-Ⅰ和IGF-ⅠR的表达与性别、年龄、分化程度无关(P>0.05);TNM分期Ⅲ+Ⅳ期胃癌组织的IGF-Ⅰ和IGF-ⅠR蛋白表达水平均高于Ⅰ+Ⅱ期(P<0.05);伴有淋巴结转移胃癌组织中的表达明显高于不伴有淋巴结转移者(P<0.05)。IGF-Ⅰ和IGF-ⅠR在胃癌组织中的蛋白表达水平呈正相关(rs=0.5824,P<0.001)。结论IGF-Ⅰ和IGF-ⅠR在胃癌组织中蛋白表达水平与胃癌TNM分期、转移能力有关,提示IGF-Ⅰ和IGF-ⅠR的高表达在胃癌的发展过程中起了重要作用。
Objective To explore the expression and significance of insulin-like growth factor-Ⅰ (IGF-Ⅰ) and (IGF- Ⅰ R) in human gastric cancer. Methods We analyzed the expression of IGF- Ⅰ and IGF- I R in 40 paired samples of tumor-free tissues and carcinomatous gastric tissues by immunohistochemistry. The relationship of expression of IGF- Ⅰ and IGF- Ⅰ R with various clinicopathological parameters were analyzed. Results As measured by immunohistochemistry, the positive rates of IGF- Ⅰ and IGF- Ⅰ R expressions in tumor tissues were 80% and 77.5%, significantly higher than that of 27.5% and 25% in tumor-free tissues, respectively (P 〈 0. 005). No correlation were found between expression levels of IGF- Ⅰ/IGF- Ⅰ R and differentiation of patients sex, age, histology (P 〉 0.05 ). The IGF- Ⅰ and IGF- ⅠR protein expressions were higher in stage Ⅲ+ Ⅳ of gastric cancer than that in stage Ⅰ +Ⅱ of gastric cancer, and were higher in gastric cancer with lymphnode metastasis than that in those without lymphnode metastasis (P 〈 0.05). There was a positive correlation between the overexpression of IGF- Ⅰ and IGF- Ⅰ R in the gastric cancer. Conclusion The over expression of IGF- Ⅰ/IGF- Ⅰ R in gastric cancer is correlated with the TNM stage and the metastasis. Our results suggest that IGF- Ⅰ/IGF- Ⅰ R plays an important role in the pathogenesis of gastric cancer.
