血浆CGRP与三叉神经痛发病关系的临床观察 被引量:4

摘要 目的:通过研究三叉神经痛患者疼痛发作时是否有降钙素基因相关肽(CGRP)的参与以及血液CGRP浓度与疼痛强度的关系,探讨诊断和描述疼痛的客观指标。方法:用放射免疫法检测46例三叉神经痛患者疼痛时患侧颈外静脉血及治疗后患侧颈外静脉血中CGRP的含量,并以35名健康成人颈外静脉血中CGRP的含量作为正常对照,对疼痛强度(NRS评分)与局部血液CGRP含量进行相关分析。结果:治疗前颈外静脉血中CGRP浓度较治疗后及正常对照组明显增多(P<0·05、P<0·01),局部血浆中的CGRP浓度与三叉神经痛痛觉强度存在正相关关系(r=0·6208P<0·01)。结论:三叉神经痛的疼痛发作确有CGRP的参与,成正相关,局部血液CGRP含量可作为一种疼痛强度的客观描述指标。 Objective : To observe whether calcitonin gene-related peptide (CGRP) participates in the attack of trigeminal neuralgia and the relation between the CGRP levels of plasma in the external jugular vein and the degree of pain, get an objective mark of pain. Methods: During the attacks of trigeminal neuralgia, CGRP levels of plasma in the external jugular vein blood ipsilateral to pain were assessed by sensitive radioimmunoassay in 46 cases, after treatment the venous blood from the external jugular vein were sampled and assessed again, the external jugular vein blood of 35 normal volunteers were collected as contrast. The linear correlation was done between the degree of pain (NRS) and the CGRP levels. Results: During the attacks of trigenfinal neuralgia the CGRP levels in the external jugular vein were significantly higher than that post - treatment and which of normal volunteers ( P 〈 0. 05 and P 〈 0. 001 ). There are positive correlation between the CGRP levels and the degree of pain (NRS) ( r = 0. 564 P 〈 0. 01 ). Conclusion: There are some relationship between the CGRP and trigeminal neuralgia, CGRP indeed participates locally in the attack of trigeminal neuralgia. The rise of the CGRP levels may regard as an objective sign to describe the degree of the pain.
