摘要
目的 探讨彩超多参数综合分析对乳腺癌与乳腺纤维腺瘤鉴别诊断的价值。方法 1 .用二维高频超声观察乳腺癌和乳腺纤维腺瘤的声像图特征、周围皮肤、皮下脂肪层及腋窝淋巴结的改变 ;2 .用彩色多普勒血流图 (CDFI)观察肿块内部血流信号 ,测量动脉血流参数 ;3.超声诊断结果与手术病理结果进行比较。结果 二维高频超声多参数分析结合 CDFI观察肿块内部血管形态及走行对乳腺癌和乳腺纤维腺瘤鉴别的敏感性为94 .9% ,CDFI血流信号分级及动脉血流参数对大于 2 cm肿块的鉴别无明显作用 (P值均大于 0 .0 5 ) ,对小于 2 cm肿块有明显作用 (P值均小于 0 .0 5 )。结论 肿块大小超声测值与临床触诊的比值、肿块形态、边界回声、肿块内部血管形态及走行是乳腺癌和乳腺纤维腺瘤鉴别诊断的重要指标。肿块周围皮肤、皮下淋巴管。
Objective To investigate the differential diagnosis value between breast carcinoma and breast fibroadenoma by comprehensive analysis of multiple parameters on color Doppler ultrasonography Methods 1 The ultrasonic characteristics of breast carcinoma and breast fibroadenoma,the skin and subcutaneous fat layers near the lump,and the subaxillary lymph nodes were detected using 2 dimensional high frequency ultrasonography(HFU);2 The signals and parameters of artery blood stream inside the lump were detected using CDFI;3 The ultrasonic diagnosis was compared with pathology.Results The sensitivity of differentiaing breast carcinoma from fibroadenoma was 94 9% throush analysis of multiple parameters on color Doppler ultrasonography and via the observation of the shape and distribution of blood vessels near the lump on CDFI The signal grading of blood stream on CDFI was significant for the differential diagnosis of the two kinds of lumps when the sizes of the lumps were≥ 2cm( P <0 05),but not significant when <2 cm( P >0 05).Conclusions The ratio of the size of lump detected by HFU to that measured by palpation,the shape of lumps,the echo at the edges,the morphology and distribution of blood vessels inside the lumps detected on CDFI are all the key criteria for differential diagnosis of breast carcinoma and fibroadenoma The ultrasonic characteristics of skin,lymph vessels,subcutaneous fat layers near the lump,and axillary lymph nodes are probably helpful for improving the differential diagnosis rate.
