摘要
目的 比较超声、钼靶摄影与近红外扫描对乳腺肿瘤的诊断 ,明确各自优势 ,提高乳腺肿瘤的诊断水平。方法 75 0例经手术、病理证实的乳腺疾病 ,其中炎症 12例 ,增生性和瘤样病变 2 2 1例 ,良性肿瘤 2 0 9例 ,恶性肿瘤 30 8例。术前经超声检查 731例 ,钼靶检查 6 0 2例 ,近红外扫描 6 2 7例。结果 超声、钼靶、近红外对乳腺疾病的检出率分别为 93.9%、5 6 .5 %、81.5 % ,三者差别显著。诊断正确率 ,除炎症三者相近 ;恶性肿瘤超声与近红外相近 ,其它均为超声高于近红外 ,近红外高于钼靶。误诊率三者相近。结论 无论良恶性乳腺疾病的诊断 ,超声优于钼靶和近红外。但其各有优缺点 ,应具体分析 ,综合应用。
Objective By comparing the diagnosresults of breast tumor by ultrasonography、mammography and near infrared light scanning,the advantages of each diagnostic method will be made clear, so as to raise the level of diagnosis for breast fumor.Methods There were 750 patients with breast diseases,of which 12 cases with mastitis,221 cases with mammary gland hyperplasia and tumor-like lesions,209 cases with benign tumor and 308 cases with malignant tumor,they were all confirmed by surgical and pathological findinges in our lospital,Among the total cases, 731 cases underwent ultrasonography,602 cases underwent mammography, and 627 cases underwent near infrared light scanning before surgery. Results The detection rates of breast disease by ultrasonography、mammography and near infrared light scanning were 93.9%、56.5% and 81.5% respectively,the difference between them was statistical significant. To various breast diseases,except mastitis’the diagnostic accurate rates, by the three methods were similar,for the malignant tumor the ultrasonography was similar to the near infrared light scanning,as for other disease, the ultrasonography was higher than the near infrared light scanning,and the latter was higher than the mammography. The misdiagnosis rates of three methods were similar.Conclusion For the diagnosis of breast diseases no matter benign or malignant,ultrasonography is better than mammography and near infrared light scanning. But they all have their own advantage and defect. In order to raise totally the level of diagnosis for breast discases, detailed and comprehensive study should be considered.
