摘要
目的探讨虹膜周边切除术对角膜内皮细胞的近期以及远期影响。方法用角膜内皮细胞分析仪对 5 4例 (34眼 )手术虹膜周边切除 (34眼 )和激光虹膜周边切除 (2 0眼 )的患眼进行内皮细胞观察。结果与术前角膜内皮细胞密度比较 ,手术组术后 1周和 1个月差异有显著性 ,术后 3个月、6个月、1 2个月差异无显著性。激光组术后 1周差异有显著性 (P <0 .0 5 ) ;其他时间段差异均无显著性 (P >0 .0 5 ) ;两种治疗方法比较 ,激光组在术后 1个月时 ,对角膜内皮的损害较手术组小 (P <0 .0 5 ) ;其他时间段两者差异无显著性 (P>0 .0 5 )。结论虹膜周边切除术简单易行 ,并发症极少 ,远期效果好 。
ObjectivesTo explore the recent and long term effects of peripheral iridectomy on the corneal endothelia.MethodsThe corneal endothelia (CE) of fifty four patients with glaucoma attack of acute angle closure were observed by CE analyzer, including 34 eyes subjected to operative peripheral iridectomy (Group A) and 20 eyes subjected to laser iridectomy(Group B) .ResultsIn Group A, corneal endothelial specular microscopy was performed before and 1 week, 1,3,6 and 12 months after surgery.The decrease in CE density one week and one month after iridectomy revealed statistically significant difference as compared with the preoperative figures (P<0.05). No significant difference existed in the decrease of CE density 3,6,12 months after iridectomy (P>0.05) . When two types of operation were compared, the damage of corneal endothelia within one month after laser iridectomy in Group B was much milder than that in Group A (P<0.05) ; there were no significant differences between CE density of both groups at other time intervals (P>0.05).ConclusionPeripheral iridectomy is a well established surgical procedure with few complication and very satisfactory long term outcome, showing certain importance for clinical application.
