摘要
观察巩膜扣带术后眼前节的远期改变及发生闭角性青光眼的危险性。方法 :应用超声生物显微镜 ( U BM)对 17例( 17眼 )裂孔源性视网膜脱离患者 ,在巩膜扣带手术前、手术后 1周及手术后 5个月的前房轴深、睫状体厚度、前房开放距离和小梁虹膜夹角进行检测。结果 :经术后 5个月随访 ,17例患者全部获得视网膜解剖复位。前房轴深 ,术后 1周与术前比明显变浅 ,术后 5个月与术前无显著性差异 ;睫状体厚度 ,术后 1周比术前明显增厚 ,术后 5个月与术前无显著性差异 ;前房角开放距离和小梁虹膜夹角 ,术后 1周比术前房角明显变窄 ,而术后 5个月与术前无显著性差异。结论 :巩膜扣带术对患眼的眼前节远期影响不明显 。
To observe the long term changes of ocular anterior segment after scleral buckling surgery and the danger of inducing angle closure glaucoma.Methods:17 patients who suffered from rhegmatogenous retinal detachment were examined with ultrasound biomicroscopy(UBM)preoperatively,1 week and 5 months after surgery.Central anterior chamber depth,ciliary thickness,anterior chamber angle opening distance(AOD)and trabecular iris angle were measured and the results were tested by the t pair test.Results:After 5 months follow up,the retina was anatomically reattached in 17 patients.Central anterior chamber depth meant(2 865±0 531)mm before surgery and (2 554±0 540)mm 1 week after surgery,the difference being statistically significant.After 5 months,the central anterior chamber depth meant(2 817±0 546))mm and had no significant differences comparing with that before surgery.Ciliary thickness meant(0 381±0 105)mm before surgery and (0 529±0 113)mm 1 week after surgery,the difference being statistically significant.After 5 months,the ciliary thickness meant(0 413±0 098)mm and had no significant differences comparing with that before surgery.AOD and trabecular iris angle menat(0 641±0 390)mm,(38 03±18 29)degree before surgery and they meant(0 436±0 271)mm,(29 76±14 84)degree 1 week after surgery,the difference being statistically significant.After 5 months,the AOD and trabecular iris angle menat(0 581±0 303)mm,(35 19±14 46)degree and the anterior chamber angle had no significant chages comparing with that before surgery.Conclusions:Scleral buckling surgery had no significant influence on ocular anterior segment in long term and didnt increase the danger of angle closure glaucoma.
