不同镇痛方法对体感诱发电位痛相关成分P_(250)-N_(350)变化的比较实验观察

Observation of the Influences of Different Analgesic Therapies on the Changes in the Pain-Related Component P_(250)-N_(350) Complex Wave of Somatosensory Evoked Potentials

摘要 目的 比较不同方法的镇痛特点。方法 采用电刺激正常人下肢胫后神经获得痛相关成分P2 50 -N350 复合波作为反映疼痛的客观指标 ,观察不同镇痛方法对痛相关成分P2 50 -N350 波幅、潜伏时的变化影响。结果 肌注镇痛药曲马多 30min ,痛相关成分P2 50 -N350 波幅下降至最低水平 ;针药复合组药物镇痛效应减弱时P2 50 -N350 波幅回升 ,给予针刺 ,波幅再次下降 ;单纯针刺组 ,痛相关成分P2 50 -N350 明显下降 ,经统计学处理差异显著 ,分别为P <0 0 0 1、P <0 0 5、P <0 0 0 1;各组潜伏时未见规律性变化。结论 针刺镇痛效应与药物镇痛效应基本一致 。 Objective To compare the characteristics of different analgesic therapies. Method The pain related component P 250 -N 350 complex wave obtained by performing electroacupuncture to the posterior tibial nerve in normal human subjects was used as the criterion of pain; and the influences of different analgesic therapies on the changes in the amplitude and latent time of the P 250 -N 350 complex wave were observed. Results It was found that 30 minutes after intramuscular injection of tramadol the wave amplitude of P 250 -N 350 reached its lowest level. In the acupuncture drug group, the wave amplitude of P 250 -N 350 increased when the effect of tramadol became weak, but it declined again when acupuncture was given; in the acupuncture group, the wave amplitude of P 250 -N 350 markedly decreased ( P<0.001, P<0.05, P<0.001 ); and in both groups regular changes in the latent time were not found. Conclusion The analgesic effects of acupuncture and those of drug therapy are basically identical, and a combined analgesic therapy of acupuncture and drug shows that acupuncture can prolong the acting time of an analgesic.
