摘要
论述了作者在心肌核素断层显像定量分析方法研究方面的工作 ,左心室模型选用Garcia等人提出的“半球加圆柱”模型 ,在算法上提出应用左心室心肌最大放射性计数值采样及Gauss -Newton最优化方法进行模型参数的拟合 .建立了适合中国人使用的正常人对照组SPECT(两种SPECT扫描仪 ,正常人例数分别为 36和 2 3)和PET( 16例正常人志愿者 )的正常值靶心图 ,该定量分析方法研究的临床应用 ( 2 2例CAD病人的定性与定量分析 )表明对心肌放射性摄取的定量分析判断与有经验的医师的定性分析判断符合率为 81.8% ,Kappa值为 0 .6 6 9,两组结果高度相关 ,显著水平p <0 .0 1.
This paper introduces the method of quantitative analysis of myocardial perfusion imaging by SPECT and PET. The paper adoptes the maximum counts of the myocardium to fit LV to a hemispheric cylindrical model under the Gauss Newton optimization method, and developes an automatic method of quantitative analysis of myocardial radionuclide tomographic images. In clinical pracfice, the data base of SPECT (36 and 23 normal Chinese subjects for two SPECT scanners) and PET (16 normal Chinese subjects) were established. We studied 22 CAD patients by this method, the agreement between the quantitative analysis and the qualitative analysis of experts by visual of myocardial images was excellent (81.8%), Kappa was 0.669. Two results were highly correlative, respectively, with significance p <0.01.\;
