摘要
目的:评价2种冠外精密附着体义齿修复牙列缺损的临床效果。方法:采用键槽缓压式(ASC-52)附着体13副,按扣式Revax system附着体10副,分别修复14例牙列缺损患者。其中肯氏Ⅰ类缺损3例,肯氏Ⅱ类缺损6例,肯氏Ⅲ类缺损5例。对临床修复效果进行分析,并与同期进行的、同类别缺损的RPD对照。结果:经6个月-5年观察,85％以上患者认为附着体义齿美观、舒适、稳固、易适应、咀嚼有力。对74颗基牙复查结果显示,基牙无叩痛、松动,无根尖周病变及牙槽骨吸收,其中16颗基牙有龈缘充血水肿现象,附着体上少量牙石沉积;对照组56颗基牙中24颗(42.86％)有龈缘充血水肿,其中12颗(21.42％)甚至有颈部继发龋发生。所有修复患者咀嚼效能测定:PAD组6个月后的咀嚼效能可达同龄正常牙列者的82.40％,5年后达正常牙列者的93.30％;RPD组6个月后达53.90％,5年后达71.90％,P<0.05。结论:冠外精密附着体义齿能达到良好的修复效果。
Objective: In this study, clinical effects of two extracoronal precision attachment prostheses were evaluated. Methods: 13 attachment of ASC-52 and 10 Revax system attachments were applied to restore 14 dentition defect cases, which consisted of 3 kennedy I defects, 6 kennedy II defects, 5 kennedy III defects. The clinical effects of two precision attachment prostheses were analyed and were compared with the RPD prostheses. Results: 14 patients with extracoronal precision attachment prostheses were followed up for 6 months to 5 yeas. More than 85% patients felt comfortable, adaptive, aesthetics and fine retention .The rechecking results of 74 abutments displayed that all the abutments were healthy without percussion pain, looseness, and disease of root tip and absorption of alveolar bone. The congestion and edema occur in gingival edge of 16 abutments, and some dental calculus deposition occured on attachments. Compared with RPD prostheses 24(42.8%)of all the 56 abutments occured the congestion and edema. 12 abutments had cervical caries.The masticatory efficiency measurement of all patients restored with extracoronal precision attachment prostheses showed that the patient masticatory efficiency achieved 82.40% of the same age healthy dentition after 6 months, and 93.30% of the same age healthy dentition after 5 years. Compared RPD prostheses 53.90% after 6 months, and 71.90% after 5 years, P < 0.05. Conclusion: The extra coronal precision attachment prostheses can achieve satisfactory effect.
