摘要 目的探讨增殖分子在甲状腺良恶性组织中表达的意义,以及在分化性腺癌中与AMES分级的相关性。方法应用免疫组织化学方法,检测术前未行特殊治疗的48例甲状腺癌和15例良性病变中P53突变蛋白及增殖细胞核抗原(PCNA)的表达。结果P53突变蛋白在良性病变中无阳性表达,恶性肿瘤中为16/48(33.3%),分化性腺癌中为12/43(27.9%),未分化癌中为4/5(80.0%)。良恶性病变中,其表达差异有显著意义(P<0.05)。但与年龄、性别、肿瘤直径、AMES分级无明显相关。PCNA表达在恶性肿瘤中明显升高。分化性腺癌中,其表达与年龄、肿瘤直径、AMES分级显著相关(P<0.01),与性别无关。随着肿瘤分化程度的降低,P53突变蛋白与PCNA表达逐渐升高,两者间呈现正性相关。结论P53突变蛋白与PCNA协同表达于甲状腺恶性肿瘤,联合测定有助于判断肿瘤恶性度及预后。AMES分级能有效地判断分化性腺癌的恶性程度。 Objective To investigate the expression of P53mutant protein in thyroid tissue,ac cess the value of AMES grade in differentiated thyroid can cinomas.Methods 48patients of thyroid cancer and 15b enign disease,without undergoing preoperative therapy,were studied for the expression of P53mutant protein and proliferating cell nuclear a ntigen(PCNA)by using immunohistochemistry methods.Results P53mutant protein was detected in no case of benign disease,16/48cases(33.3%)of thyroid cancer.12/43(27.9%)of the differentiated thyroid cance r and 4/5(80.0%)of the undifferentiated cancer expr essed P53mutant protein.There was obviou s difference between cancer and beni gn disease(P<0.05).The expression of the P53mutant protein had no correlation with age?sex?tumor size and AMES grade.PCNA expre ssion was significantly higher in ma lignant tissue than in benign disease.In different iated cancer,PCNA expression had correlation with age,tumor size and AM ES grade,expect sex.In thyroid tissue,with the progression of the tumor,P53mutant protein expression had the same extense w ith the PCNA.Conclusion The expression of the P53mutant protein`PCNA has relationship with developi ng progression of thyroid cancer.They are available markers in reflec ting the biological behavior and pro gnosis of thyroid cancer.AMES grade can assess the ma-lignant degree of differentiated th yroid cancer.
