摘要
绝经后骨质疏松症(postmenopausal osteoporosis,PMOP)作为一种中老年女性中常见的全身性骨病,已经对其健康造成严重影响。清代医家叶天士在前人的理论基础上及自身临证经验下总结提出“女子以肝为先天”的理论,强调肝对于女子生理病理的重要性。本文从“女子以肝为先天”理论出发,将PMOP的病机概括为肝肾阴虚、肝郁血瘀、肝血不足等,认为可从荣养肝阴、补肾填精,疏肝解郁、化瘀通络,养肝补血、滋养骨骼分而治之,体现从肝论治PMOP的重要性,以期为PMOP的中医药治疗提供理论指导,为临床防治提供新的思路。
Postmenopausal osteoporosis(PMOP),as a common systemic bone disease,has had a serious impact on the health of middle-aged and elderly women.The Qing Dynasty physician Ye Tianshi summarized and proposed the theory that"theliver serves as the congenital root for females"based on the theoretical foundation of predecessors and his own clinical experience,emphasizing the importance of the liver in women's physiology and pathology.Based on"theliver serves as the congenital root for females",this article summarized their pathological connotations as liver and kidney yin deficiency,liver depression and blood stasis,and liver blood deficiency.It was believed that the treatment could be divided into liver and kidney tonifying and nourishing liver yin,soothing the liver and dredging collaterals and regulating liver qi,supplementing liver and nourishing marrow,and nourishing liver blood.This reflected the importance of treating PMOP from the perspective of the liver,in order to provide theoretical guidance for the treatment of PMOP with traditional Chinese medicine and new ideas for clinical prevention and treatment.
