摘要
目的:研究探讨国医大师刘敏如教授治疗不孕的方药规律和临床经验。方法:分析刘敏如教授治疗不孕症的门诊病历处方,分别运用SPSS 24及IBM SPSS Modeler 18统计软件进行频次、聚类、因子及关联规则分析,探讨刘老治疗不孕症用药规律,探究其经验特色。结果:纳入处方158份,平均每方使用中药15味。使用频次、频率比较多的有11味中药:山萸肉、白芍药、淮山药、枸杞、熟地黄、菟丝子、川牛膝、当归、淫羊藿、鹿角胶和龟板胶。使用性平的药物频次最多(占31%)。归经的分类以肝经和肾经使用频次最多(分别约30%和约20%)。另外种类分类以补益类药物使用频次最多(占54%)。结论:刘老临床治疗不孕,以其首倡的“脑-肾-天癸-冲任-胞宫生殖轴”的学术观点为指导,强调“肾气为根,保阴为本”。对治疗不孕症的经验特色是:衷中参西,审因为先;补肾益精,贯彻始终,常用左归丸;精血同源,佐以养肝补血;病证结合,随证化裁;顺应月经,择期论治;用药平和,通调气血及提倡男女同治。
Objective:To discuss the medication rule and characteristics of National Chinese Medicine Grandmaster,Professor Liu Minru in the treatment of infertility.Methods:After collecting the prescriptions used by Prof.Liu Minru from 2007 to 2016 in the cases of infertility in the outpatient department,use SPSS 24 statistical software to do the frequency,class and factor analysis;use IBM SPSS Modeler 18 for analysis of association rules for medications.Results:The data analysis included a total of 158 prescriptions,with an average of 15 medications per prescription.From the analysis of the frequency of drug use,the first 11 Chinese herbal medicines with higher frequency were:Cornus officinalis,White peony root,Chinese yam,Goji berries,Rehmannia(cooked),Cuscuta seed,Achyranthes bidentata,Angelica sinensis,Epimedium,Deer antler glue,and Turtle shell glue.The medicinal properties are most commonly neutral(accounting for 31%).The meridians they belong to are primarily the Liver meridian(accounting for 30%)and the Kidney meridian(accounting for 20%).The most common category is tonifying herbs(54%).Concolusion:Under the academic perspective of“Brain–Kidney–Tian gui–Chong ren–Uterus Reproductive Axis”,she emphasizes that in the clinical treatment of infertility,“Kidney Qi is the root,and nourishing the Yin is fundamental”.The distinctive features of infertility treatment are:integrating traditional Chinese medicine with Western medicine,thoroughly analyzing the cause;tonifying the Kidney and replenishing Essence,maintaining consistency throughout the treatment,often using Zuogui Wan;Essence and Blood share the same origin,with additional focus on nourishing the Liver and supplementing Blood;combining disease and syndrome,adjusting treatment according to symptoms;aligning treatment with the menstrual cycle,selecting appropriate timing;using mild medications,regulating Qi and Blood;and advocating for simultaneous treatment of both partners.
