摘要
目的:探讨肺癌化疗所致周围神经病变的潜在分型,并分析不同分型的影响因素。方法:采用目的抽样法,选取2023年1月—2024年6月于复旦大学附属中山医院进行紫杉烷类化疗的315例肺癌病人为调查对象。运用一般资料调查表、化疗所致周围神经病变量表(CIPNAT)进行问卷调查。使用R语言4.3.2软件对潜在剖面类型进行统计分析,通过单因素分析和多元Logistic回归探究不同分型的影响因素。结果:肺癌化疗所致周围神经病变可分为两个潜在剖面,分别为低反应组(81.59%)和高反应组(18.41%)。单因素分析表明,两组性别、失眠、高血压、化疗次数、血红蛋白水平、居住方式、疲乏、化疗用药比较差异有统计学意义(P<0.05)。多元Logistic回归分析显示,性别、失眠、高血压、化疗次数是不同肺癌化疗所致周围神经病变分型的影响因素。结论:肺癌化疗所致周围神经病变病人群体存在显著异质性。医护人员应关注肺癌化疗病人周围神经病变情况,依据不同分型的影响因素制订个性化的干预策略,实施精准的治疗与护理,持续改善肺癌化疗病人神经病变症状,提升其生活质量。
Objective:To explore the potential subtypes of peripheral neuropathy caused by chemotherapy in lung cancer and analyze the influencing factors of different subtypes.Methods:A purposive sampling method was used to select A total of 315 lung cancer patients who underwent taxane chemotherapy at Affiliated Zhongshan Hospital of Fudan University from January 2023 to June 2024 as the study subjects.A questionnaire survey was conducted using a General Information Questionnaire and the Chemotherapy-Induced Peripheral Neuropathy Assessment Tool(CIPNAT).Statistical analysis of potential profile types was performed using R language 4.3.2 software.Univariate analysis and multivariate logistic regression were used to explore the influencing factors of different subtypes.Results:Peripheral neuropathy caused by chemotherapy in lung cancer could be divided into two potential profiles:a low-response group(81.59%)and a high-response group(18.41%).Univariate analysis showed statistically significant differences in gender,insomnia,hypertension,number of chemotherapy sessions,hemoglobin levels,living arrangements,fatigue,and the use of single-drug/double-drug chemotherapy(P<0.05).Multivariate logistic regression analysis revealed that gender,insomnia,hypertension,and number of chemotherapy sessions were influencing factors for different subtypes of peripheral neuropathy induced by lung cancer chemotherapy.Conclusion:There is significant heterogeneity among patients with peripheral neuropathy induced by lung cancer chemotherapy.Healthcare professionals should pay attention to the peripheral neuropathy status of lung cancer chemotherapy patients,develop personalized intervention strategies based on the influencing factors of different subtypes,implement precise treatment and nursing care,continuously improve the neuropathy symptoms of lung cancer chemotherapy patients,and enhance their quality of life.
