摘要
患者,女,53岁,农民,因右手臂多发性暗红色斑块、结节、囊肿、瘢痕1年多来就诊,皮肤表面脱屑,无特别瘙痒、刺痛等自觉症状,未见明显水疱、大疱、脓疱、糜烂、溃疡、裂隙、浸渍等特殊皮疹。根据皮肤组织病理、真菌培养等结果,诊断为甄氏外瓶霉致皮肤暗色丝孢霉病。参照药敏结果,给予口服伊曲康唑胶囊、外用萘替芬酮康唑乳膏,治疗8周后,患处皮疹明显变平,破溃处愈合。
The patient,a 53-year-old female,was a farmer with multiple dark red plaques,nodules,cysts and scars on the right arm for more than a year.There was no desquamation on the skin surface,no special symptoms such as itching and tingling.There were no obvious blisters,pustules,erosion,ulcers,fissures,impregnation and other special rashes.Based on the results of skin histopathology,fungal culture,etc.,the diagnosis was made of dermatophytosis caused by Exophiala jeanselmei,According to the results of antifungal sensitivity,itraconazole capsules and topical naltifen ketoconazole cream were administered.After 8 weeks of treatment,the affected area was significantly flattened and the broken area healed.
