摘要
甄氏外瓶霉(Exophiala jeanselmei)是一种机会致病菌,常发生于免疫功能受损患者。本文报道1例免疫功能正常个体右手食指感染甄氏外瓶霉致的病例。患者,女,59岁。3个月前外伤后右手食指皮肤出现红色丘疹。组织病理示化脓型肉芽肿。PAS染色示真皮组织内串珠样菌丝结构。培养真菌经鉴定为甄氏外瓶霉。最终诊断为甄氏外瓶霉致皮肤暗色丝孢霉病。伊曲康唑联合手术切除治疗后痊愈。
Exophiala jeanselmei is an opportunistic pathogen that commonly occurs in immunocompromised patients.This report presents a case of Exophiala jeanselmei infection in the right index finger of an immunocompetent individual.The patient,a 59-year-old female,developed a red papule on the skin of her right index finger following trauma three months prior.Histopathological examination revealed a pyogenic granuloma.PAS staining demonstrated beaded hyphal structures within the dermal tissue.Fungal culture identified the pathogen as Exophiala jeanselmei.The diagnosis of cutaneous phaeohyphomycosis due to Exophiala jeanselmei was made.The patient was successfully treated with a combination of itraconazole and surgical excision.
