摘要
梅尼埃病(Meniere's disease,MD)是以内淋巴积水(endolymphatic hydrops,EH)为核心病理特征的内耳疾病,其发病机制复杂,常规西医治疗效果有限且复发率高。中医药以其多靶点、整体调节的特点,在改善MD症状方面显示出应用潜力。本文旨在系统综述中医药调控MD相关关键信号通路的研究进展。综述内容表明,中药单体、复方及针灸可通过调控核因子-κB(NF-κB)、丝裂原活化蛋白激酶(MAPK)、精氨酸血管加压素-水通道蛋白2(AVP-AQP2)及磷脂酰肌醇3-激酶/蛋白激酶B(PI3K/Akt)等信号通路,进而发挥抑制炎症反应、调节内耳水液代谢、减轻氧化应激与保护神经元等多重作用,以缓解EH及相关症状。当前研究仍存在作用机制阐释不清、高质量临床证据匮乏等问题。未来需结合现代技术深入阐明中医药干预MD的分子机制,并开展严谨的临床研究,为开发新型治疗策略提供科学依据。
Meniere's disease(MD)is an inner ear disorder characterized by the core pathological feature of endolymphatic hydrops(EH).It has a complex pathogenesis,and conventional Western medical treatments offer limited efficacy with a high recurrence rate.Traditional Chinese medicine(TCM),with its multi-target and holistic regulatory characteristics,demonstrates potential in improving the symptoms of MD.This article aims to systematically review the research progress on the TCM modulation of key signaling pathways related to MD.The review indicated that active monomers from Chinese herbs,compound formulas,and acupuncture can modulate signaling pathways such as nuclear factor-kappa B(NF-κB),mitogen-activated protein kinase(MAPK),arginine vasopressin-aquaporin 2(AVP-AQP2),and phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt).Thus,TCM ameliorates EH and its associated symptoms via multiple effects,including inhibiting inflammatory responses,regulating inner ear fluid metabolism,alleviating oxidative stress,and providing neuroprotection.However,current research is still limited by insufficient mechanistic elucidation and a scarcity of high-quality clinical evidence.Future research should integrate modern technologies to further elucidate the molecular mechanisms of TCM interventions in MD and conduct rigorous clinical studies,which will provide a scientific basis for the development of novel therapeutic strategies.
