摘要
癌因性疲乏是由肿瘤发生发展及治疗引发的癌症患者躯体、情感和认知等多维度的主观疲乏感,具有持续性、非特异性及难以通过常规休息缓解的典型特征。目前,癌因性疲乏的发病机制尚未完全明确,涉及炎性反应、三磷酸腺苷(Adenosine Triphosphate,ATP)代谢异常、神经内分泌紊乱及肿瘤发生发展及治疗等。线粒体作为细胞能量代谢的核心细胞器,其功能异常与癌因性疲乏的发生发展密切相关。线粒体通过氧化磷酸化生成ATP,为脏腑生理活动提供能量支持。研究发现,中医脾与线粒体的生理功能和病理表现颇为相似,而线粒体功能障碍可能是脾虚型癌因性疲乏发生发展的关键。当机体处于脾虚状态时,其运化功能减退,导致气血生成不足、水液代谢失序,这些病理改变与线粒体数量减少、超微结构异常及功能损伤呈现正相关关系,从而导致能量代谢障碍,引发疲乏等症状。这种线粒体功能改变不仅是脾虚证候的重要病理基础,也可能通过影响能量供应加剧癌因性疲乏的临床症状。脾为后天之本,主运化水谷精微并将其输布至周身以滋养脏腑百骸,与线粒体的能量代谢功能存在显著相似性。本研究拟从线粒体角度探讨其与癌因性疲乏发生发展的关系,重点分析线粒体功能障碍与脾虚证的相关性,如通过调节线粒体功能、改善能量代谢纠正脾虚状态,为癌因性疲乏患者提供更有效的治疗策略,提高其生活质量。
Cancer-related fatigue(CRF)is a multidimensional subjective fatigue of physical,emotional,and cognitive exhaustion experienced by cancer patients induced by the occurrence,progression and therapeutic interventions of tumors.It is characterized by persistence and non-specificity,and is difficult to be relieved after conventional rest.The pathogenesis of CRF is incompletely elucidated currently,involving multi-factor such as inflammatory responses,adenosine triphosphate(ATP)metabolic abnormalities,neuroendocrine dysregulation,and the occurrence,progression and treatment of tumor.Mitochondria are the central organelles of cellular energy metabolism,and their dysfunction plays a pivotal role in the occurrence and progression of CRF.Mitochondria generate ATP via oxidative phosphorylation,providing energy support for physiological activities of visceral organs.Research has found significant parallels between the physiological functions and pathological manifestations of the spleen in traditional Chinese medicine(TCM)and mitochondrial dynamics,and mitochondrial dysfunction might be a critical factor in the occurrence and progression of spleen deficiency type CRF.In state of spleen deficiency,impaired digestive and transportative functions of the body lead to deficient Qi(气)and blood production and disrupted fluid metabolism.These pathological changes correlate positively with mitochondrial depletion,ultrastructural abnormalities,and functional impairment,resulting in energy metabolic disorders and fatigue symptoms.Such mitochondrial function alteration is not only an important pathological basis of spleen deficiency syndrome,but may also exacerbate the clinical symptoms of CRF by affecting energy supply.In TCM theory,the spleen is recognized as the foundation of postnatal vitality,responsible for metabolizing nutrients and distributing essential substances to nourish all organs and tissues,which is a process obviously analogous to mitochondrial energy metabolism.In this study,the relationship between mitochondria and the occurrence and development of CRF is explored from the perspective of mitochondria,focusing on the correlation of mitochondrial dysfunction and spleen deficiency syndrome,such as correcting spleen deficiency state by regulating mitochondrial function and ameliorating energy metabolism,so as to provide more effective therapeutic strategies for CRF patients,ultimately improving their quality of life.
