摘要
目的:分析高龄重症患者急性肾损伤的影响因素。方法:于北京急性肾损伤课题组(BAKIT)数据库收集2012年3—8月北京市28家三级医院重症监护室(ICU)收治的544例患者(≥80岁)的临床资料,根据急性肾损伤患病情况将患者分为非急性肾损伤组、急性肾损伤组,比较两组临床资料,分析高龄重症患者急性肾损伤的影响因素。结果:544例患者发生急性肾损伤333例(61.2%),急性肾损伤患者死亡149例,病死率为44.7%。合并糖尿病、合并慢性肾功能不全、患有脓毒症、发生休克是高龄重症患者急性肾损伤的独立危险因素(P<0.05)。结论:高龄重症患者急性肾损伤的患病率及病死率较高,临床医师应针对相关危险因素尽早采取防治措施,使患者获得最佳转归。
Objective:To analyze the influencing factors of acute kidney injury in elderly critically ill patients.Methods:The clinical data of 544 patients(≥80 years old)admitted to the intensive care units(ICU)of 28 tertiary hospitals in Beijing from March to August 2012 were collected in the database of the Beijing Acute Kidney Injury Research Group(BAKIT).The patients were divided into the non-acute kidney injury group and the acute kidney injury group according to the incidence of acute kidney injury.The clinical data of the two groups were compared.The influencing factors of acute kidney injury in elderly critically ill patients were analyzed.Results:Among 544 patients,333 cases(61.2%)developed acute kidney injury,and 149 patients with acute kidney injury died,with a case fatality rate of 44.7%.Diabetes mellitus,chronic renal insufficiency,sepsis and shock are independent risk factors for acute kidney injury in elderly critically ill patients(P<0.05).Conclusion:The prevalence and mortality of acute kidney injury in elderly critically ill patients are relatively high.Clinicians should take preventive and therapeutic measures as early as possible against relevant risk factors to achieve the best outcome for patients.
