以活性微生物群为导向的饮食干预对特应性皮炎发病及严重程度的影响:一项多中心临床研究

The Effect of Live Microbiota-Oriented Dietary Intervention on the Onset and Severity of Atopic Dermatitis:A Multicenter Clinical Study

摘要 目的探讨以活性微生物群为导向的饮食干预对特应性皮炎(AD)患者发病及严重程度的影响,为AD的非药物治疗提供科学依据。方法选取2025年1月—2025年10月赣州市3家三甲医院就诊的100例AD患者,采用随机数字表法分为干预组和对照组,每组50例。对照组维持常规饮食习惯,干预组在常规饮食基础上补充活性微生物食物(含益生元、益生菌、合生元),干预周期6周。比较两组治疗前、治疗2周末、6周末的特应性皮炎积分指数(SCORAD)、特应性皮炎控制工具评分(ADCT)、瘙痒数字评定量表(NRS)及皮肤病生活质量指数(DLQI),并统计临床疗效及复发率。结果干预6周末,干预组SCORAD评分(18.76±4.23)分、ADCT评分(5.32±1.89)分、NRS评分(1.65±0.78)分、DLQI评分(6.24±2.15)分,均显著低于对照组(P<0.05);干预组总有效率86.00%,显著高于对照组的64.00%(χ^(2)=7.523,P=0.006);干预组复发率8.89%,显著低于对照组的29.41%(χ^(2)=5.472,P=0.019)。结论以活性微生物群为导向的饮食干预可有效改善AD患者的临床症状,提高治疗效果,降低复发率,且安全性高,值得临床推广。 Objective To explore the effect of live microbiota-oriented dietary intervention on the onset and severity of atopic dermatitis(AD),and to provide a scientific basis for the non-pharmacological treatment of AD.Methods A total of 100 AD patients who visited 3 Grade A tertiary hospitals in Ganzhou from January 2025 to October 2025 were selected and divided into an intervention group and a control group using a random number table method,with 50 cases in each group.The control group maintained a regular diet,while the intervention group supplemented with live microbiota-containing foods(including prebiotics,probiotics and synbiotics)on the basis of a regular diet,with an intervention cycle of 6 weeks.The Scoring Atopic Dermatitis(SCORAD),Atopic Dermatitis Control Tool(ADCT),Numerical Rating Scale(NRS)for pruritus and Dermatology Life Quality Index(DLQI)of the two groups were compared before treatment,at the end of the 2nd week and the 6th week of treatment.Meanwhile,the clinical efficacy and recurrence rate were statistically analyzed.Results At the end of the 6-week intervention,the SCORAD score(18.76±4.23),ADCT score(5.32±1.89),NRS score(1.65±0.78)and DLQI score(6.24±2.15)of the intervention group were significantly lower than those of the control group(P<0.05).The total effective rate of the intervention group was 86.00%,which was significantly higher than that of the control group(64.00%)(χ^(2)=7.523,P=0.006).The recurrence rate of the intervention group was 8.89%,which was significantly lower than that of the control group(29.41%)(χ^(2)=5.472,P=0.019).Conclusion Live microbiota-oriented dietary intervention can effectively improve the clinical symptoms of AD patients,enhance the therapeutic effect,reduce the recurrence rate with high safety,and is worthy of clinical promotion.
