摘要
目的 分析不同体质量指数(BMI)的多囊卵巢综合征(PCOS)患者糖脂代谢指标、胰岛素抵抗及卵巢储备功能。方法 选择该院2024年1—12月收治的187例患者,根据BMI将患者分为正常组、超重组与肥胖组,比较3组PCOS患者的糖脂代谢指标、胰岛素抵抗及卵巢储备功能。采用Spearman相关分析PCOS患者BMI与糖脂代谢指标、胰岛素抵抗及卵巢储备功能的关系。结果 187例PCOS患者中BMI正常94例,超重40例,肥胖53例,分别纳入正常组、超重组与肥胖组。3组腰围、臀围、腰臀比、收缩压(SBP)、舒张压(DBP)比较,差异均有统计学意义(P<0.05),正常组腰围、臀围、SBP、DBP小于超重组与肥胖组(P<0.05),超重组小于肥胖组(P<0.05),正常组腰臀比小于肥胖组(P<0.05)。3组空腹胰岛素(FINS)、空腹血糖(FPG)、甘油三酯(TG)水平及稳态模型胰岛素抵抗指数(HOMA-IR)、卵巢储备功能比较,差异均有统计学意义(P<0.05),正常组患者FINS、FPG水平以及HOMA-IR低于超重组与肥胖组(P<0.05),超重组低于肥胖组(P<0.05),正常组高密度脂蛋白胆固醇(HDL-C)水平高于超重组与肥胖组(P<0.05),TG、总胆固醇(TC)水平低于超重组与肥胖组(P<0.05),正常组及超重组卵巢储备功能低下患者比例少于肥胖组(P<0.05)。Spearman相关分析结果显示:PCOS患者BMI与FINS、FPG、TG、TC、HOMA-IR均呈正相关(r=0.508、0.261、0.490、0.279、0.913,P<0.001),与HDL-C呈负相关(r=-0.216,P=0.003),与低密度脂蛋白胆固醇无显著相关性(r=0.066,P=0.372)。结论 随着PCOS患者BMI的增加,其发生糖脂代谢异常、胰岛素抵抗及卵巢储备功能下降的风险更高。
Objective To investigate the glucose and lipid metabolism indicators,insulin resistance and ovarian reserve function in patients with polycystic ovary syndrome(PCOS)across different body mass index(BMI)categories.Methods A total of 187 patients admitted to the hospital from January to December 2024 were enrolled.According to BMI,patients were classified into a normal-weight group,an overweight group and an obese group.Indices of glucose and lipid metabolism indicators,insulin resistance and ovarian reserve function were compared among the three PCOS groups.Spearman′s rank correlation was used to assess the relationships between BMI and glucose and lipid metabolism indicators,insulin resistance and ovarian reserve function in PCOS patients.Results Among the 187 patients with PCOS,94 were classified as normal weight,40 as overweight,and 53 as obese.Significant differences were observed in waist circumference,hip circumference,waist-to-hip ratio(WHR),systolic blood pressure(SBP)and diastolic blood pressure(DBP)among the three groups(P<0.05).Waist circumference,hip circumference,SBP and DBP were lower in the normal-weight group than those in the overweight and obese groups(P<0.05),and were lower in the overweight group than those in the obese group(P<0.05).WHR was lower in the normal-weight group than that in the obese group(P<0.05).Levels of fasting insulin(FINS),fasting plasma glucose(FPG),triglycerides(TG)and homeostasis model assessment of insulin resistance(HOMA-IR),as well as ovarian reserve function,differed significantly among the three groups(P<0.05).FINS,FPG,and HOMA-IR were lower in the normal-weight group than those in the overweight and obese groups(P<0.05),and were lower in the overweight group than those in the obese group(P<0.05).High-density lipoprotein cholesterol(HDL-C)was higher in the normal-weight group than that in the overweight and obese groups(P<0.05),whereas TG and total cholesterol(TC)were lower(P<0.05).The proportion of patients with decreased ovarian reserve function was lower in the normal-weight and overweight groups than that in the obese group(P<0.05).Spearman correlation analysis showed that BMI was positively correlated with FINS,FPG,TG,TC and HOMA-IR(r=0.508,0.261,0.490,0.279 and 0.913 respectively;all P<0.001),negatively correlated with HDL-C(r=-0.216,P=0.003),and not significantly correlated with low-density lipoprotein cholesterol(r=0.066,P=0.372).Conclusion A higher BMI in PCOS patients is associated with not only an increased risk of abnormal glucose and lipid metabolism and insulin resistance,but also a greater likelihood of diminished ovarian reserve function.
