摘要
早发性卵巢功能不全是一种常见的女性内分泌疾病,其发病率随着社会压力的增加而逐渐上升,且发病年龄趋向年轻化。该病病因多样,包括遗传、免疫等因素,但具体的发病机制尚不清楚。中医治疗早发性卵巢功能不全主要采用中药内治法和外治法,如针灸、穴位埋线等,这些方法在改善患者症状、恢复卵巢储备功能方面显示出一定疗效。中医学认为,早发性卵巢功能不全可归属于“经水早断”“不孕”“月经病”等范畴,治疗时多采用辨证论治、调周法及联合疗法。中药治疗早发性卵巢功能不全的具体方案通常取决于患者的具体证候,如肾阴虚证、心肾不交证、肾虚肝郁证、肾阳虚证等。西医治疗早发性卵巢功能不全则主要依据患者的年龄和需求来选择不同的方式,常用的治疗手段包括激素补充治疗,对有生育需求的患者可能会采用辅助生殖技术。激素补充治疗可以缓解低雌激素症状,并对心血管疾病和骨质疏松起到预防作用。中西医结合治疗早发性卵巢功能不全旨在发挥两种医学的优势,降低复发率,改善患者的生活质量,并减少低雌激素对女性的长期影响。本研究主要从早发性卵巢功能不全的病因、发病机制、中医治疗、西医治疗、中西医结合治疗等五个方面进行阐述,以期为临床诊疗早发性卵巢功能不全提供新思路。
Premature ovarian insufficiency(POI)is a common female endocrine disease.Its incidence rate gradually rises with the increase of social pressure,and the age of onset tends to be younger.The etiology of this disease is diverse,including genetic factors,immune factors,etc.,but the specific pathogenesis is still unclear.Traditional Chinese medicine mainly adopts Chinese medicinal material internal treatment method and external treatment methods such as acupuncture and moxibustion,and acupoint catgut embedding to treat POI.These methods have shown certain efficacy in ameliorating patients’symptoms and restoring ovarian reserve function.In traditional Chinese medicine,POI can be classified into categories such as“early menstrual cessation”,“infertility”,and“menstrual disease”.The treatment methods mainly include syndrome differentiation and treatment,menstrual cycle adjustment,and combined therapy.The specific treatment plan for POI in traditional Chinese medicine is usually personalized according to the specific syndrome of patients,such as kidney Yin(阴)deficiency syndrome,heart-kidney disharmony syndrome,kidney deficiency and liver depression syndrome,and kidney Yang(阳)deficiency syndrome.The treatment of POI in Western medicine mainly depends on the patients’age and needs to select different methods.Common treatment approaches include hormone replacement therapy and so on.Assisted reproductive technology may be used for patients with fertility needs.Hormone replacement therapy can alleviate symptoms of low estrogen levels and has a preventive effect on cardiovascular disease and osteoporosis.The treatment of POI in the integrative medicine aims to leverage the advantages of both medical approaches,decrease recurrence rates,improve quality of life of patients,and decrease the long-term effects of low estrogen levels on patients.In this study,POI is mainly elaborated from five aspects of the etiology,pathogenesis,traditional Chinese medicine treatment,Western medicine treatment,and treatment in the integrative medicine,in order to provide new ideas for clinical diagnosis and treatment of POI.
