摘要
本文以产后抑郁(PPD)的中医药治疗机制为切入点,系统探讨其多层次、多靶点、多趋势的治疗特点,旨在挖掘中医药“辨证论治”的循证医学证据。基于文献复习及PPD相关基因靶点分析,进一步探索PPD的中医药防治策略。本文发现,PPD的中医药治疗以“调、补”为核心,辅以“理、行”之法;其作用机制主要涉及调控MAPK、AKT/mTOR、ERK-CREB等关键信号通路,平衡下丘脑-垂体-性腺(肾上腺、甲状腺、肠)轴功能,调节脑-肠神经递质、激素水平及免疫状态,从而在多层次、多通路上发挥治疗效应。建议在PPD早期即开展中医药干预,注重“形”“神”并调、“身”“心”同治,为深化中医药防治PPD的临床实践提供参考依据。
This paper focuses on the mechanism of traditional Chinese medicine(TCM)in treating postpartum depression(PPD),systematically investigating its multi-level,multi-target and multi-directional therapeutic characteristics.The aim is to explore the evidence-based medical basis of"treatment based on syndrome differentiation"in TCM.Based on a review of the literature and analysis of PPD-related gene targets,this study further explores prevention and treatment strategies for PPD from the perspective of TCM.The paper reveals that the TCM approach to treating PPD is centered on"regulating and tonifying",supplemented by methods for"rectifying and activating".The mechanisms of action primarily involve the regulation of key signaling pathways,such as MAPK,AKT/mTOR and ERK-CREB,balancing of the hypothalamic-pituitary-gonadal(adrenal,thyroid,gut)axis function,and modulation of brain-gut neurotransmitters,hormone levels and immune status.Thereby,therapeutic effects are exerted across multiple levels and pathways.It is recommended to initiate TCM interventions in the early stages of PPD,emphasizing the"simultaneous regulation of physical and mental states"and the"integrated treatment of body and mind,"thereby providing a reference for enhancing the clinical practice of TCM in preventing and treating PPD.
