摘要
基于中医阴阳五行学说,结合体脏合痹理论,阐释风湿免疫病中体与脏的生理、病理联系及治疗策略。阴阳学说强调体与脏基于互根互用的动态依存关系形成内外联系,在病理表现为体痹及脏、脏痹及体,与现代医学免疫-代谢-炎症失衡机制高度契合。五行学说以生克制化揭示脏体间动态循环特征,在病理表现为五行乘侮与母子同病,与现代医学多系统交互网络失衡相符。治疗层面,提出体脏同治指导下的阴阳自和(以体养脏、调脏安体)及体脏调和指导下的五行制化(培土生金、金水相生、培土治水等治法),为风湿免疫病的系统性治疗提供参考。
Based on the yin-yang and five-element theory of traditional Chinese medicine(TCM),combined with the theory of body-viscus impediment,this paper explained the physiological and pathological connections between the body and viscus in rheumatic and immunological diseases,along with corresponding therapeutic strategies.The yin-yang theory emphasizes the dynamic interdependence between body and viscus through mutual rooting and mutual use,forming internal-external connections.Pathologically,this manifests as body impediment affecting viscera,and visceral impediment affecting body,which align closely with immune-metabolic-inflammatory imbalance mechanisms in modern medicine.The five-element theory reveals the dynamic cyclical characteristics between viscus and body through the principles of generation and restriction.Pathologically,the five elements manifests over-restriction and counter-restriction each other,as well as disease involving both the mother-organ and child-organ,which correspond to the imbalance of multi-system interactive networks in modern medicine.For treatment,this article proposes that yin-yang self-harmonization guided by simultaneous treatment of body and viscus(nourishing viscus through the body,regulating viscus to stabilize the body),and five-element restriction and generation guided by harmonizing body and viscus(methods such as nourishing earth to generate metal,metal-water mutual generation,and nourishing earth to treat water disorders),so as to provide reference for systemic treatment of rheumatic and immunological diseases.
