摘要
目的 基于R语言对《临证指南医案》中治疗妇人产后病方剂进行数据挖掘,探讨叶天士治疗产后病用药规律。方法 收集《临证指南医案·产后门》中药处方,采用Excel2021统计药物性味归经,使用Origin2024软件绘制雷达图。使用R4.3.3对中药进行频次分析、关联规则及聚类分析。结果 共纳入处方111首,涉及药物162味,药物总频次为757。频次前10位药物为茯苓、人参、当归、茯神、枸杞子、生地黄、白芍、小茴香、桂枝、炙甘草;药性以温为主,其次为平、寒、热,药味以甘为主,苦、辛次之,归经以肾经为主,心经、脾经、肝经次之;药物功效以补虚类为主,其次为利水渗湿药、清热药、温里药。关联规则显示,常用药物配伍组合共52组,聚类分析得到6组核心处方。结论 叶天士治疗产后病重补益气血,善辨证论治、灵活加减,用药讲求寻源探本、精简得当,重气血而慎滋腻,值得临床借鉴。
Objective To conduct data mining on the prescriptions for treating postpartum disease in women in Lin Zheng Zhi Nan Yi An based on R language;To explore the medication law of Ye Tianshi in treating postpartum disease.Methods Chinese materia medica prescriptions was collected from Lin Zheng Zhi Nan Yi An·Chan Hou Men.Excel 2021 was used to count properties,tastes and meridian tropism of the included Chinese materia medica.Origin 2024 was used to draw a radar chart.R 4.3.3 was used to perform frequency analysis,association rules and clustering analysis on the included Chinese materia medica.Results After screening,a total of 111 prescriptions were included,involving 162 kinds of Chinese materia medica,with a total frequency of 757 times.The top 10 kinds of Chinese materia medica with the highest frequency were Poria,Ginseng Radix et Rhizoma,Angelicae Sinensis Radix,Poria cum Radix,Lycii Fructus,Rehmannine Radix,Crataegi Fructus,Foeniculi Fructus,Cinnamomi Ramulus,and Glycyrrhizae Radix et Rhizoma Praeparata cum Melle;the main property was warm,followed by neutral,cold and hot,the main taste was sweet,followed by bitter and pungent,and the main meridian was kidney meridian,followed by heart meridian,spleen meridian and liver meridian;the main efficacy of drugs was to supplement deficiency,followed by diuretics,heat clearing drugs,and warming drugs.According to the association rules,there were a total of 52 commonly used drug combinations,and clustering analysis obtained 6 core prescriptions.Conclusion Ye Tianshi treats postpartum disease by tonifying qi and blood.He is good at syndrome differentiation and treatment,flexible in addition and subtraction,and emphasizes seeking the source and exploring the essence of medication.His medication is concise and appropriate,emphasizes qi and blood while being cautious in nourishing and avoiding greasiness,which is worthy of clinical reference.
