Diagnostic techniques and control strategies of chikungunya fever

摘要 基孔肯雅热是由基孔肯雅病毒(CHIKV)引起的急性蚊媒传染病,临床症状以发热、皮疹和关节疼痛为主,在热带及亚热带地区广泛流行。系统发育分析将CHIKV划分为3种地理关联基因型:西非型(WAf)、东/中/南非型(ECSA)和亚洲型(Asian genotype)。我国至少在16个省份监测到基孔肯雅热的输入性病例,其中云南、浙江和广东省还发生过输入性病例引起的本地流行,病例多由东南亚与南亚地区输入,发病具有明显的聚集性,流行株主要是东/中/南非型印度洋进化谱系和亚洲基因型。基孔肯雅热的实验室检测主要依赖血清学检测和分子生物学检测,综合使用可提高诊断准确性。2025年7月以来,我国广东省发生输入性病例引起的基孔肯雅热暴发流行,截至7月22日确诊病例3 195例,为加强疫情防控应对措施,本文对基孔肯雅热的诊断技术与防治策略进行综述,为疫情防控提供参考。 Chikungunya fever is an acute mosquito-borne infectious disease caused by chikungunya virus(CHIKV),characterized by fever,rash,and joint pain.It is widely prevalent in tropical and subtropical regions.Phylogenetic analysis divides CHIKV into three geographically associated genotypes:West African genotype(WAf),East/Central/South African genotype(ECSA),and Asian genotype.At least 16 provinces in China have detected imported cases of chikungunya fever,among which Yunnan,Zhejiang,and Guangdong have also experienced local epidemics caused by imported cases.The cases are mostly imported from Southeast Asia and South Asia,and the incidence has obvious clustering.The epidemic strains are mainly East/Central/South African Indian Ocean evolutionary lineages and Asian genotypes.The laboratory testing of chikungunya fever mainly relies on serological testing and molecular biology testing,and the comprehensive use can improve diagnostic accuracy.Since July 2025,there has been an outbreak of chikungunya fever caused by imported cases in Guangdong Province,China.As of July 22,there were 3195 confirmed cases.In order to strengthen epidemic prevention and control measures,this article summarizes the diagnostic techniques and control strategies for chikungunya fever,providing reference for epidemic prevention and control.
