摘要
目的:探究排卵障碍性不孕症患者采用自拟补肾疏肝方联合针灸治疗的效果及其超声参数的变化。方法:前瞻性选取九江市妇幼保健院2022年7月—2024年12月收治的排卵障碍性不孕症患者共80例,采用随机数字表法分为观察组(n=40)与对照组(n=40)。两组患者均接受常规西药治疗,观察组则给予自拟补肾疏肝方联合针灸治疗。观察两组3个月经周期治疗后效果,治疗前及治疗结束后月经来潮第3天血清性激素[雌二醇(E_(2))、卵泡刺激素(FSH)、黄体生成素(LH)]水平,治疗前及治疗结束后月经来潮第10天超声检查结果[子宫内膜厚度、子宫动脉搏动指数(PI)、阻力指数(RI)],记录两组排卵率、妊娠率及不良反应发生率。结果:观察组治疗总有效率较对照组更高,差异有统计学意义(P<0.05);两组治疗后月经来潮第3天血清E_(2)水平及第10天子宫内膜厚度较治疗前升高,且观察组较对照组更高,差异有统计学意义(P<0.05);两组治疗后月经来潮第3天血清FSH、LH水平及第10天子宫动脉PI、RI较治疗前降低,且观察组较对照组更低,差异有统计学意义(P<0.05);观察组排卵率、妊娠率较对照组更高(P<0.05),但两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:自拟补肾疏肝方联合针灸治疗排卵障碍性不孕症患者效果显著,有助于纠正性激素水平,恢复子宫内膜厚度及子宫动脉血流参数,提高排卵率及妊娠率,且治疗方案安全。
Objective:To explore the effect of self-made Bushen Shugan Decoction combined with acupuncture in the treatment of patients with anovulatory infertility and the changes in ultrasound parameters.Method:A total of 80 patients with anovulatory infertility admitted to Jiujiang Maternal and Child Health Hospital from July 2022 to December 2024 were prospectively selected,and they were divided into the observation group(n=40)and the control group(n=40)by random number table method.Two groups of patients were treated with conventional western medicine,while the observation group was given self-made Bushen Shugan Decoction combined with acupuncture and moxibustion.The efficacy after 3 menstrual cycles of treatment,the levels of serum sex hormones[estradiol(E_(2)),follicle stimulating hormone(FSH),luteinizing hormone(LH)]before treatment and on the 3rd day of menstruation after the end of treatment,the ultrasound examination results[endometrial thickness,uterine artery pulsatility index(PI),resistance index(RI)]before treatment and on the 10th day of menstruation after the end of treatment in two groups were compared,the ovulation rate,pregnancy rate and incidence of adverse reactions of two groups were recorded.Result:The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05);the serum E_(2) level on the 3rd day of menstruation after treatment and endometrial thickness on the 10th day of menstruation after treatment in two groups were risen compared with those before treatment,and the two indicators in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05);the levels of serum FSH and LH on the 3rd day of menstruation after treatment and uterine artery PI and RI on the 10th day of menstruation after treatment in two groups were significantly reduced compared with those before treatment,and the indicators in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).The ovulation rate and pregnancy rate in the observation group were higher than those in the control group(P<0.05),but there was no statistical difference in the incidence of adverse reactions between groups(P>0.05).Conclusion:Self-made Bushen Shugan Decoction combined with acupuncture and moxibustion has significant efficacy in the treatment of patients with anovulatory infertility,and it is helpful to correct the sex hormones,restore the endometrial thickness and uterine artery blood flow parameters,and enhance the ovulation rate and pregnancy rate,and the treatment regimen is safe.
