摘要
目的:评价宫安颗粒联合地屈孕酮片治疗围绝经期异常子宫出血的疗效。方法:选择2021年10月-2023年10月山东省寿光市人民医院收治的98例围绝经期异常子宫出血患者,采用随机数字表法分为治疗组和对照组,每组49例,筛除脱落病例后,治疗组有46例患者,对照组有45例患者。对照组予地屈孕酮片口服,治疗组予宫安颗粒联合地屈孕酮片口服。连续治疗3个周期并随访6个月。对比两组患者止血情况、子宫内膜厚度、中医证候积分、总有效率、激素水平及血清生长因子水平,统计两组不良反应发生率。结果:治疗后两组患者出血控制时间、完全止血时间、子宫内膜厚度均降低,且治疗组低于对照组(P<0.05)。治疗组临床总有效率较对照组高,且差异有统计学意义(P<0.05)。两组治疗后促卵泡生成素(Follicle Stimulating Hormone,FSH)、雌二醇(Estradiol,E2)、促黄体生成素(Luteinizing Hormone,LH)水平均下降,且治疗组低于对照组(P<0.05)。治疗后两组血清生长因子指标均升高,且治疗组高于对照组(P<0.05)。结论:宫安颗粒联合地屈孕酮片治疗围绝经期异常子宫出血可以有效止血,降低子宫内膜厚度,改善中医证候,调节血清激素水平和生长因子水平,且不良反应低,是一种安全有效的治疗方法。
Objective:To evaluate the clinical effect of the Gongan granules(宫安颗粒)combined with dydrogesterone tablets on abnormal uterine bleeding during perimenopausal period.Methods:A total of 98 patients with abnormal perimenopausal uterine bleeding admitted to Shouguang People’s Hospital of Shandong Province from October 2021 to October 2023 were selected.Based on the random number table method,there were 46 cases in the treatment group and 45 cases in the control group.The control group was treated with oral dydrogesterone tablets,while the treatment group was treated with the Gongan granules combined with oral dydrogesterone tablets.The patients were treated for 3 consecutive cycles and followed up for 6 months.The hemostasis,endometrial thickness,TCM syndrome score,total effective rate,hormone level,and growth factor levels were compared between the two groups,and the adverse reaction rate in the two groups was counted.Results:After treatment,the bleeding control time,bleeding stop time and endometrial thickness in the two groups were decreased,and the value in the treatment group was lower than the control group(P<0.05).The clinical effective rate in the treatment group was higher than that in the control group,and the variance was statistically significant(P<0.05).After treatment,the levels of follicle stimulating hormone,estradiol and luteinizing hormone in the two groups were decreased,and those in the treatment group were lower than the control group(P<0.05).After treatment,the serum growth factor indexes in the two groups were increased,and in the treatment group was higher than the control group(P<0.05).Conclusion:The Gongan granules combined with dydrogesterone tablets in the treatment of abnormal uterine bleeding during perimenopausal period can effectively stop bleeding,reduce endometrial thickness,ameliorate TCM syndrome,regulate serum hormone levels and growth factor levels,with low adverse reactions.It is a safe,effective and novel treatment method.
