摘要
目的探讨消瘕汤结合地诺孕素对子宫腺肌症患者性激素水平及卵巢功能的影响。方法收集2021年1月—2022年12月在医院接受诊治的肾虚血瘀型子宫腺肌症患者90例,按照随机数字表法分为A组(地诺孕素治疗组)与B组(消瘕汤结合地诺孕素治疗组),各45例。A组给予地诺孕素治疗,B组给予消瘕汤结合地诺孕素治疗。比较两组治疗效果、中医症状积分、性激素水平、血清指标。结果B组治疗有效率明显高于A组(P<0.05)。治疗后,B组月经周期、经量、经行腹痛、精神抑郁、胸胁胀满、舌紫黯积分均明显低于治疗前(P<0.05),且B组明显低于A组(P<0.05)。治疗后,B组睾酮(testosterone,T)、促卵泡素(follicle stimulating hormone,FSH)、促黄体生成素(luteinizing hormone,LH)水平均明显低于治疗前(P<0.05),且B组明显低于A组(P<0.05),雌二醇(estradiol,E_(2))水平明显高于治疗前(P<0.05),且B组明显高于A组(P<0.05)。治疗后,B组基质金属蛋白酶-9(matrix metalloproteinase-9,MMP-9)、血管内皮生长因子(vascular endothelial growth factor,VEGF)、基质金属蛋白酶-2(matrix metalloproteinase-2,MMP-2)水平均明显低于治疗前(P<0.05),且B组明显低于A组(P<0.05)。结论消瘕汤结合地诺孕素治疗子宫腺肌症患者具有较好临床疗效,有利于减缓子宫腺肌症疾病进展,调节患者性激素水平,改善卵巢功能,有效缓解患者临床症状。
Objective To explore the effect of Xiaojia Decoction(消瘕汤)combined with denogestrin on sexual hormone levels and ovarian function in patients with adenomyosis.Method Ninety patients with kidney deficiency and blood stasis type of adenomyosis who were treated in the hospital from January 2021 to December 2022 were collected and randomly divided into A group and B group,with 45 patients in each group.Group A was treated with dinoprotein,while Group B was treated with Xiaojia Decoction combined with dinoprotein.The treatment effects,traditional Chinese medicine symptom scores,sex hormone levels and serum indicators were compared between the two groups.Results The effective rate of treatment in Group B was significantly higher than that in Group A(P<0.05).After treatment,the scores of menstrual cycle,menstrual volume,menstrual abdominal pain,mental depression,chest and flank fullness and tongue purple dark in Group B were significantly lower than those before treatment(P<0.05),and the scores of Group B were significantly lower than those of Group A(P<0.05).After treatment,the levels of testosterone(T),follicle stimulating hormone(FSH)and luteinizing hormone(LH)in Group B were significantly lower than those before treatment(P<0.05),and the levels of Group B were significantly lower than those of Group A(P<0.05).The level of estradiol(E2)was significantly higher than that before treatment(P<0.05),and the level of Group B was significantly higher than that of Group A(P<0.05).After treatment,the levels of matrix metalloproteinase-9(MMP-9),vascular endothelial growth factor(VEGF)and matrix metalloproteinase-2(MMP-2)in Group B were significantly lower than those before treatment(P<0.05),and the levels of group B were significantly lower than those of Group A(P<0.05).Conclusion The combination of Xiaojia Decoction and dinoregestone has good clinical efficacy in treating patients with adenomyosis,which is beneficial for slowing down the progression of adenomyosis,regulating the levels of sexual hormones in patients,improving ovarian function and effectively alleviating clinical symptoms.
