摘要
目的:对新生儿经外周置入中心静脉导管(PICC)管理的最佳证据进行汇总,并评价其应用效果。方法:计算机检索国内外数据库中关于新生儿PICC管理的相关文献,并汇总形成最佳证据,据此制订循证实践方案;经过基线审查、障碍因素分析等构建最佳证据应用策略。比较证据应用前后护士对新生儿PICC理论知识知晓水平和临床审查指标执行情况,以及患儿并发症发生情况。结果:证据应用前后护士对PICC相关理论知识的问卷得分分别为(60.00±14.55)分和(75.45±11.18)分,护士临床审查指标的符合率也显著改善,差异均有统计学意义(P<0.05)。且证据应用后新生儿PICC相关并发症(静脉炎、导管堵塞和导管异位)发生率降低,差异有统计学意义(P<0.05)。结论:基于循证证据的新生儿PICC管理培训可以提高临床护士的专业知识和临床实践能力,并促进其护理质量的提升。未来应继续探索和优化护理流程及培训机制,以进一步改善新生儿护理的安全性和效果。
Objective:To summarize the best evidence for the management of peripherally inserted central catheters(PICC)in neonates,and evaluate its effectiveness.Methods:The relevant article on neonatal PICC management in domestic and foreign databases was searched by computer,and the best evidence was summarized to formulate evidence⁃based practice plan.Build the best evidence application strategy through baseline review and obstacle factor analysis.The theoretical knowledge level of neonatal PICC and the implementation of clinical review indicators,as well as the occurrence of complications in children before and after the application of evidence were compared.Results:Before and after the application of evidence,the questionnaire scores of nurses on PICC⁃related theoretical knowledge were(60.00±14.55)points and(75.45±11.18)points.The conformity rate of nurses'clinical review indicators was also significantly improved respectively,with statistical significance(P<0.05).After the application of evidence,the incidence of neonatal PICC⁃related complications(phlebitis,catheter blockage and catheter ectopic)decreased,and the difference was statistically significant(P<0.05).Conclusions:Evidence⁃based training in neonatal PICC management can improve the expertise and clinical practice of clinical nurses and promote the improvement of their quality of care.In the future,we should continue to explore and optimize the nursing process and training mechanism to further improve the safety and effectiveness of neonatal care.
